急性双侧肾皮质坏死的MRI特征

MR Features of Acute Bilateral Renal Cortical Necrosis

目的探讨急性肾皮质坏死(ARCN)的MRI表现。方法回顾性分析5例经穿刺活检或临床确诊的ARCN患者的临床及影像学资料。4例行MRI平扫,1例行平扫+增强;2例行CT平扫并CT随访。结果MRI扫描5例ARCN均表现为T2WI上双肾皮质及肾柱呈线状低信号。1例行MRI增强可观察到双侧肾脏皮质无强化,肾包膜、外层髓质强化。结论MRI是诊断ARCN安全有效的方法。T2WI上皮质及肾柱线状低信号、显著的皮髓质信号差异是ARCN的特征性MRI表现。

Objective To evaluate the MR feature of acute renal cortical necrosis ( ARCN) . Methods Five cases ofdiagnosed ARCN by clinical or pathologic methods were retrospectively analyzed. 4 cases had MR plain scan and one casehad plain scan + enhancement. 2 of 5 cases were followed up by CT plain scan. Results Dark signal intensity rim in-volving the inner cortex and column of Bertin was noted on T 2 -weighted image ( 5/5) . Unenhanced cortex and enhancedcapsule,outer medulla were noted in CEMR imaging ( 1/1) . Conclusion Plain MR is safe and promising in diagnosingARCN. Dark signal intensity rim involving the inner cortex and column of Bertin in T2-weighted images is the characteris-tic finding of ARCN.
