神经梅毒患者伴发记忆功能损害与炎症因子和脂类水平的关系

摘要 【目的】探讨神经梅毒患者伴发记忆功能损害与炎症因子和血脂水平的关系。【方法】对114例梅毒患者(A组)和120例正常体检人群(B组)的心理学背景、记忆能力进行比较。根据梅毒患者记忆功能有无损害分为Al组(有损害)和A2组(无损害),比较两组患者的炎症因子水平与血脂水平。【结果】A组受试者简易精神状态量表(MMSE),数字广度测试(DS),听觉词语学习测试(AVLT)测试评分明显低于B组.且差异均有显著性(P<0.05);言语流畅性测试(VFT)的评分与B组比较差异无显著性(P>0.05)。A组基于时间的记忆得分和基于事件的记忆得分均较B组明显降低,且差异具有显著性(P<0.05)。Al组患者肿瘤坏死因子-α(TNF-α)和白介素-6(IL-6)水平明显高于A2组;而IL-10的水平明显低于A2组(P<0.05)。A1组患者的血清中总胆固醇(TC),三酰甘油(TG)和低密度脂蛋白胆固醇(LDL-C)水平明显高于B组及A2组(P<0.05)。三组对象HDL-C、ApoA、ApoB水平比较差异均无显著性(P>0.05)【结论】梅毒患者较正常人群的精神和记忆能力受到明显损害.伴发记忆功能损害与炎症因子水平和脂类物质代谢具有相关性。 [Objective]To explore the association of memory impairment in neurosyphilis patients with-levels of inflammatory eytokines and serum lipids.[MethodslA total of 114 patients with neurosyphilis were in group A and 120 healthy volunteers as controls were in group B.The psychological background and memory ability of the two groups were compared.According to the memory function impairment,114 syphilis patients were divided into group A1(with memory impairment)and group A2(without impairment).Levels of inflammatory cytokines and serum lipids were compared in both groups.[Results]The scores of mini-mental state scale (MMSE),Digital Spanning Test (DS)and Auditory Words Learning Test (AWLT)of group A were significantly lower than those of group B (P<0.05).There was no significant difference in the scores of verbal fluency test (VFT)between group A and group B (P >0.05).The time-based and event-based memory scores of group A were significantly lower than those of group B,and the difference was statistically significant (P<0.05).The levels of TNF-α and IL-6 in group Alwere significantly higher than those in group A2;while the level of IL-10 of A1 group was signifi- cantly lower than that of A2 group (P<0.05).Serum total cholesterol (TC),glyceryi triglyceride (TG)and low density lipoprotein cholesterol (LDL-C)levels were significantly higher in group A1 than in group B and A2(P<0.05).There were no significant differences in the levels of HDL-C,ApoA,and ApoB among all groups (P> 0.05).[ConclusionlThe mental and memory abilities of syphilis patients are significantly impaired compared to the normal population,and memory impairment is associated with inflammatory factor levels and lipid metabolism.
