术前应用盐酸氨溴索对老年妇科腹腔镜手术患者术中肺通气功能的影响 被引量:1

摘要 【目的】探讨术前应用盐酸氨溴索对老年妇科腹腔镜手术患者术中肺通气功能的影响。【方法】68例老年腹腔镜下行子宫切除术患者随机分为两组,各34例,观察组于气管插管后10min应用盐酸氨溴索,对照组应用同剂量生理盐水。比较两组用药前(T1)、气腹后5min(T2)、30min(T3)、60min(T4)及气管拔管前(T5)时的心率(HR)、平均动脉压(MAP)、呼气末二氧化碳分压(PETCO2)、血氧饱和度(SpO2)、气道阻力(Raw)、平台压(Pplat)、肺顺应性(Compl)、气道峰压(Ppeak)。【结果】与T1相比,两组T2~T4时Ppeak与Pplat均明显升高,T2~T5时Compl均下降,T3时Raw升高,且差异具有显著性(P<O.05);与对照组相比,T3~T4时观察组Ppeak与Pplat降低,T2~T5时Compl均升高,T4时Raw降低,且差异有显著性(P<0.05)。与T1相比,两组T2时HR加快、MAP升高,PET CO2在T2~T3时升高,且差异有显著性(P<0.05);两组各时间点HR,MAP和PETCO2组间比较差异无显著性(P>0.05)。【结论】于术前应用盐酸氨溴索可改善老年妇科腹腔镜患者术中肺通气功能,并可有效降低气道阻力,提高肺顺应性,稳定术中循环及呼吸指标,值得推广应用。
