多种中药注射液联合G-CSF治疗化疗后白细胞减少症的实验研究

Experimental Research on Treating Mice with Chemotherapy-related Leukopenia by Several Compound Traditional Chinese Medicine Injections

目的:通过实验研究比较5种常用中药注射液联合重组人粒细胞集落刺激因子(G-CSF)治疗化疗相关白细胞减少症的疗效及安全性。方法:将120只小鼠随机分为A^H组,共8组,A组为正常组,B组为模型组,C组为G-CSF治疗组,D^H组分别为多种中药注射液联合G-CSF治疗组。采用环磷酰胺腹腔注射法制备化疗后白细胞减少症小鼠模型,用G-CSF腹腔注射治疗C组小鼠,分别用5种中药注射液(康艾、复方苦参、参附、参麦和生脉注射液)联合G-CSF治疗D^H组小鼠,治疗完成后检测小鼠血常规及肝肾功能指标;计算重要脏器及胸腺指数,并比较各组间脏器病理变化,评价各治疗组的疗效和安全性。结果:各中药注射液联合G-CSF治疗组的白细胞计数较B组明显升高(P<0.01)。G-CSF可使模型小鼠的血红蛋白进一步降低(P<0.05),F组小鼠血红蛋白最高,与B组无明显差异(P>0.05)。E组ALT及F组AST较C组升高(P<0.05)。与B组相比,C、F、G、H组小鼠肺脏指数升高(P<0.05或P<0.01),D组肝脏指数升高(P<0.05),肾脏指数下降(P<0.05),E组脾脏指数降低(P<0.05)。F组肝脏病理损害程度大于B组和C组(P<0.05)。结论:多种中药注射液联合G-CSF治疗化疗后白细胞减少症小鼠在某些方面有一定帮助,参麦注射液有轻微提升血红蛋白的作用,康艾注射液对肾脏毒性可能有一定缓解作用,但总体来说在疗效方面较单用G-CSF并无明显优势,甚至有增加不良反应的风险。

Objective :To compare the effects of five compound traditional Chinese medicine (TCM)injections versus granulocyte -colony stimulating factor(G -CSF)for treating mice with chemotherapy -related leukopenia and their impacts upon major organs and the thymus.Methods :Chemotherapy -related leukopenia model mice were induced by cyclophosphamide.Using G -CSF as positive control,five compound TCM injections including Kang'ai,Kushen,Shenfu, Shenmai and Shengmai Injections were separately intraperitoneally injected to treat the mice for 7 days.Serum indexes including ALT,AST,BUN and CR were measured.Furthermore,we calculated mass indexes of major organs and thymus and compared their pathological changes.Results :The WBC count was significantly increased in all TCM -treated groups (P <0.01),but was lower than that of the G -CSF group(P <0.05).In addition,all TCM Injections and G -CSF were effective on increasing blood platelets(P <0.05).Compared to the model group,the AST levels were slightly increased in Kushen -treated (P =0.036)and Shenfu -treated (P =0.008)groups,whereas spleen indexes declined (P < 0.05)and lung indexes increased(P <0.01)in all TCM -treated groups.Except for Kushen Injection,liver pathologi-cal scores were significantly lower in other TCM -treated groups(P <0.05).Conclusions:All five TCM injections were effective for treating chemotherapy -related leukopenia.Furthermore,all of them could more or less increase blood platelets.
