妊娠期血小板减少症与妊娠合并特发性血小板减少性紫癜患者妊娠结局的临床分析

Clinical analysis of pregnancy outcome in patients with gestational thrombocytopenia and pregnancy with idiopathic thrombocytopenic purpura

目的 探讨妊娠期血小板减少症(GT)与妊娠合并特发性血小板减少性紫癜(ITP)对妊娠结局的影响。方法 回顾性分析41例妊娠期血小板减少症和24例妊娠合并特发性血小板减少性紫癜患者的临床资料。结果 妊娠合并ITP患者出现血小板重度减少、妊娠期贫血、产后出血、血制品输注的比率明显高于GT患者,两组在发病年龄、孕期临床症状、分娩孕周、新生儿早产率、新生儿Apgar评分等方面无明显差异。结论 妊娠合并ITP患者较GT患者围产期并发症及风险高,但两类患者及其新生儿总体预后良好。

Abstract:Objective To investigate the effect of gestationalthrombocytopenia (GT)and pregnancy with idiopathic thrombocytopenic purpura(ITP)on pregnancy outcome.Methods The clinical data of 41 GT cases and 24 cases of preg- nancy with ITP were retrospectively analyzed.Results Thepregnancy with ITP group showed remarkably higher rates of severe thrombocytopenia,anemia,postpartum hemorrhage and blood transfusion than the GT group (P <0.05),while the incidence of mild thrombocytopenia was significantly higher in the GT group than that in thepregnancy with ITP group (P <0.05).There were significant differences between the two aroups in the first time to find platelet decreases,themean minimum value ot platelets Ounng pregnancy,the presefice of uterine fatigue-during delivery,and the mean value ofplatelets one week after delivery (P<0.05).There were no significant differences between the two groups in the onset of age,pregnancy bleeding symptoms,gestational weeks,and neonatal Apgar score (P >0.05).Conclusions The incidence of perinatal complications and risks in pregnantpatientswith ITPare higher than those in patients with GT.Howev- er,both types of the patients and their newborns show generally good prognosis.
