滁州市结核病耐药性基线调查结果 被引量:1

Results of baseline survey on drug resistance of tuberculosis in Chuzhou City

摘要 目的获得滁州市耐药结核病疫情流行特征,为制定当地结核病防治工作规划提供依据。方法收集2015年9月16日—2016年8月31日期间滁州市3个耐药基线调查点涂阳肺结核病例痰标本,对分离出的190株结核分枝杆菌菌株采取比例法测定药物敏感性,获取耐药菌株的耐药谱并计算耐药率。结果滁州市涂阳肺结核患者结核分枝杆菌总耐药率达27.37%、耐多药率为6.32%,其中初治肺结核总耐药率为21.88%、耐多药率为2.50%;复治肺结核总耐药率为56.67%,耐多药率为26.67%。复治肺结核患者总耐药率和耐多药率明显高于初治肺结核患者,差异均有统计学意义(χ^2=15.38、24.94,均P<0.05)。结论滁州市耐药结核病疫情防控形势严峻,今后需加强对复治涂阳肺结核病例等重点人群耐多药结核病监测工作。 [Objective]To obtain the epidemic characteristics of drug-resistant tuberculosis in Chuzhou City,provide basis for formulating local tuberculosis control plan.[Methods]The sputum specimens of smear positive pulmonary tuberculosis were collected from three drug-resistant baseline survey points in Chuzhou City from September 16,2015 to August 31,2016. The proportional method was applied to detect drug sensitivity for 190 strains of Mycobacterium tuberculosis. The spectrum of resistant strains was obtained to calculate the resistance rate.[Results]The overall drug-resistant rate and multidrug resistance rate of Mycobacterium tuberculosis among smear positive pulmonary tuberculosis patients in Chuzhou City were 27.37% and 6.32% respectively. The overall drug-resistant rate and multidrug resistance rate of new tuberculosis cases were 21.88% and 2.5% respectively,while that of re-treatment cases was 56.67% and 26.67% respectively. The overall drug-resistant rate and multidrug resistance rate of newcases were higher than those of re-treatment cases,and the differences were statistically significant(χ^2=15.38,24.94, all P<0.05).[Conclusion]The prevention and control on epidemic situation of drug-resistant tuberculosis is severe in Chuzhou City. It is necessary to strengthen the monitoring of multidrug resistance tuberculosis in key population such as re-treatment smear positive patients.
