《中国心力衰竭诊断和治疗指南2018》是在2104年指南的基础上结合新的循证医学证据,对心衰的分类、预防、诊断、治疗和综合管理等均作了全新阐述和推荐。更新亮点包括提出射血分数中间值的心衰和急性心衰的临床分型,提出急、慢性心衰的诊断和治疗流程,推荐血管紧张素受体脑啡肽酶抑制剂和希氏束起搏。新指南重点突出心衰的综合管理和心衰随访,强调心衰的预防。新指南制订过程中充分检索和评估国内心衰研究,地高辛推荐级别和高原心脏病、老年心衰等特殊类型心衰的管理具有“中国特色”。

2018guidelines for the diagnosis and treatment of heart failure in China are based on the 2014 guidelines in China and evidence-based,especially new evidence.The aim of this document is to provide practical,evidence-based guidelines for the diagnosis and treatment of heart failure.The highlights include the definition of heart failure with mid-range ejection fraction (HFmrEF),the new clinical classification for acute heart failure,the new algorithm for the diagnosis and treatment of acute and chronic heart failure are recommended,recommendation for angiotensin receptor-nepfilysin inhibitor(ARNI)and recommendation for His bundle pacing (HBP)Chinese clinical trials in heart failure are carefully evaluated and considered.Chinese characteristics include high altitude heart disease,heart failure in elderly patients,and the strength of the recommendation of digoxin.
