护士对食管癌手术患者出院后支持性照护需求评估的现状调查

Supportive care needs assessment for postoperative esophageal cancer patients after discharge

目的 :调查护士对食管癌手术患者出院后支持性照护需求评估的现状。方法 :使用自行设计的调查问卷,对全国18所三级甲等医院胸外科护士268名进行调查;对19名患者及20名护理专家进行访谈。结果 :87.7%的护士通过直接询问患者近况来评估食管癌手术患者出院后的支持性照护需求;96.6%的护士认为每次评估,都会忽略患者的一些需求;多数护士仅依据工作经验或科室自制清单评估患者需求,评估结果与患者实际的需求不一致;45.1%的护士认为依据现有评估方式,出院后支持性照护质量不高。结论 :目前护士对食管癌手术患者出院后支持性照护需求评估方法较主观,缺乏规范的、有针对性的专用评估量表。

Objective: To investigate how patients with esophageal cancer discharged from hospital after surgery are assessed regarding their supportive care needs. Methods: Totally 268 nurses from 18 tertiary Grade A hospitals in China were investigated with self-designed questionnaire. Results: About 87.7% nurses used direct inquires about recent status for supportive care needs. About 96.6% nurses believed that they would ignore some patients' needs after every assessment. Most nurses assessed patient needs only according to their clinical experience or a department's self-designed checklist and the supportive care needs of patients which most nurses think are not consistent with the actual needs of patients themselves. About 45.1% nurses believed that the existing evaluation methods could reduce supportive care quality for postoperative esophageal cancer patients. Conclusion: The present assessment methods of supportive care needs for postoperative esophageal cancer patients after discharge are subjective. There are no standardized scales targeting supportive care needs assessment for postoperative esophageal cancer patients after discharge.
