经阴道彩色多普勒超声对异位妊娠及宫内早孕妊娠黄体的鉴别诊断

Differential Diagnosis of Transvaginal Color Doppler Ultrasound in Corpus Luteum of Ectopic Pregnancy and Early Intrauterine Pregnancy

目的研究经阴道彩色多普勒超声对异位妊娠及宫内早孕妊娠黄体的鉴别诊断。方法随机选择2017年2月至2017年12月于我院进行早孕检查的50例宫内妊娠孕妇作为早孕组,选择同期我院收治的异位妊娠孕妇33例作为异位妊娠组,两组孕妇均行经阴道彩色多普勒超声检查。观察并比较两组的黄体超声分型情况,以及黄体平均直径、阻力指数、收缩期峰值流速。结果两组孕妇的黄体超声分型均以低回声型为主,两组孕妇的各分型所占比例无统计学差异(P>0.05)。两组孕妇的黄体平均直径、收缩期峰值流速无统计学差异(P>0.05),早孕组的黄体阻力指数显著低于异位妊娠组,差异具有统计学意义(P <0.05)。结论经阴道彩色多普勒超声可通过妊娠黄体的阻力指数对异位妊娠和宫内早孕妊娠进行鉴别,为临床诊断提供可靠依据。

Objective To study the differential diagnosis of transvaginal color Doppler ultrasound in corpus Inteum of ectopic pregnancy and early intrauterine pregnancy.Methods 50cases of intrauterine pregnant women Who received examination from February 2017to December 2017in our hospital were randomly selected as early pregnancy group,and 33cases of pregnant women with ectopic pregnancy treated in our hospital were randomly selected as ectopic pregnancy group.Both groups received transvaginal color Doppler ultrasound examination.The luteal ultrasonographic typing,and the average diameter,resistance index,peak systolic velocity of the corpus luteum were observed and compared between the two groups.Results The corpus luteum classification of the two groups was mainly low echo type;No significant difference was found between the two groups in the proportions of each type (P>0.05).No significant difference was found between the two groups in the average diameter and peak systolic velocity of the corpus luteum (P >0.05),while the resistance index of the corpus luteum of the early pregnancy group was significantly lower than that of the ectopie pregnancy group (P <0.05). Conclusions Transvaginal color Doppler ultrasound can differentiate ectopic pregnancy from early intrauterine pregnancy by resistance index of corpus luteum,which provides reliable basis for clinical diagnosis.
