局部应用氨甲环酸减少全髋关节置换手术出血的疗效观察 被引量:3

摘要 目的分析全髋关节置换术(THA)中局部应用氨甲环酸(TXA)减少患者术中、术后出血的效果。方法 90例行THA治疗的患者随机均分为两组。观察组术中局部应用TXA,对照组替换为等量的生理盐水,比较两组的出血情况和并发症。结果观察组的术中出血量、术后出血量以及输血量均显著低于对照组(P <0.05)。术后1天、 3天,两组的凝血指标APTT、 PT比较无统计学差异(P>0.05),观察组的血红蛋白水平显著高于对照组(P <0.05)。观察组的并发症发生率为2.22%,低于对照组的15.56%(P <0.05)。结论行THA治疗的患者局部应用TXA,可减少术中出血量、术后出血量以及输血量,术后并发症少。 Objective To analyze the effect of local apphcatlon of tranexamlc acid (TXA)to reduce mtraoperattve aria postoperative bleeding of total hip ag_hroplasty (THA).Methods 90patients with THA were randomly divided into two groups equally.The observation group received local application of TXA during operation,and the control group received same amount of NS for replacement during operation.The bleeding situation and complications were compared between the two groups.Results The intraoperative and postoperative blood loss,and blood transfusion volume of the observation group were significantly lower than those of the control group (P<0.05).1d and 3d after surgery,no statistical difference was found between the two groups in the APTT or PT (P>0.05),while the hemoglobin level of the observation group was significantly higher than that of the control group (P<0.05).The incidence of complications of the observation group was 2.22%,significantly lower than 15.56%of the control group (P <0.05).Conclusions The local application of TXA in patients with THA can reduce the intraoperative and postoperative blood loss,and blood transfusion volume,with few postoperative complications.
