浙江省德清县农村社区成人超重和肥胖流行情况及其影响因素分析

Investigation on epidemiological status of overweight and obesity and their influencing factors among rural adults in Deqing County,Zhejiang

目的了解浙江省德清县农村社区成人超重和肥胖流行情况及影响因素。方法随机整群抽取浙江省德清县8个乡镇,选取≥18周岁户籍居民为研究对象,于2006—2008年、2011—2012年和2013—2014年分3阶段完成横断面调查。调查内容包括问卷调查、体格检查和实验室检查,用Logistic回归分析超重/肥胖的影响因素。结果2006—2008年、2011—2012年和2013—2014年德清县农村成人实际调查人数依次为6562、11 763和10 904人;超重率依次为22. 23%、29.97%和28.18%(标化率依次为19.95%、28.25%和26.10%),呈上升趋势;肥胖率依次为3. 15%、3.67%和4.46%(标化率分别为3.43%、2.77%和3.92%),亦呈上升趋势(P<0.01)。Logistic多因素回归显示,影响德清农村社区成人超重/肥胖的因素有年龄、务农和自评家庭收入。年龄方面,与18～39岁相比,40～59岁的aOR=1.528, 60～79岁的aOR=1.404;≥80岁的aOR=1.106;与不务农相比,务农的aOR=0.906;自评家庭收入方面,与自评家庭收入低相比,自评家庭收入中的aOR=1.017,自评家庭收入高的aOR=1.247。结论德清县农村社区成人超重和肥胖较常见且呈现上升趋势,年龄、务农和收入是其重要影响因素,有必要在当地积极采取控制超重/肥胖措施。

Objective To explore the epidemiological status of overweight and obesity and their influencing factors among rural adults in Deqing County of Zhejiang Province.Methods Random cluster sampling was used to select 8 rural communities in Deqing,China,and a cross-sectional survey was Conducted in 2006-2008,2011-2012 and 2013- 2014,respectively.Data of questionnaires,physical examination and laboratory tests were collected.Logistic regression was applied to explore the influencing factors of overweight/obesity.Results Totally,6562 eligible subjects were investigated in 2006-2008,11763 in 2011-2012,and 10904 in 2013-2014.The ratios of overweight were 22.23%,29.97%and 28.18%(age-standardized ratios were 19.95%,28.25% and 26.10%,respectively)in 2006-2008,2011-2012and 2013-2014,respectively.The ratios of obesity were 3.15%,3.67% and 4.46%(age-standardized ratios were 3.43%, 2.77%and 3.92%,respectively)in 2006-2008,2011-2012and 2013-2014,respectively.The prevalence of overweight and obesity significantly increased during three investigations (P <0.01).Logistic multivariate regression analysis showed that age,farming and self-rated family income were the influencing factors of overweight/obesity in rural community of Deqing.In terms of age,compared with 18-39years old,aOR of 40-59years old was 1.528,aOR of 60-79years old was 1.404,aOR of above 80years old was 1.106.Compared with non-farming,aOR of farming was 0.906.In terms of self-rated household income,aOR of medium self-rated household income was 1.017,and aOR of high self-rated household income was 1.247.Conclusion The overweight and obesity was prevalent and there was a rising trend over years among Deqing rural adults.Age,occupation and family income level were important influencing factors for overweight and obesity.It should be taken to control the overweight and obesity for rural adults.
