血清CRP与PCT及白细胞计数在糖尿病足感染中的临床价值探讨

Clinical value of serum CRP,PCT and WBC count in diabetic foot infection

目的探讨血清C-反应蛋白(CRP)、降钙素原(PCT)、白细胞计数对糖尿病足感染病情的判定。方法对95例糖尿病足感染患者根据病情严重程度进行分组,检测患者治疗前后血清中CRP、PCT水平及白细胞计数。进行糖尿病足感染患者截趾的危险因素分析。结果血中WBC计数、血清CRP及PCT水平的各组间表现为重度感染组>中度感染组>轻度感染组,各组间比较差异有统计学意义(P<0.05);截趾组溃疡程度明显深于非截趾组,发生肢体缺血比率、低蛋白血症比率高于非截趾组,经比较差异有统计学意义(P<0.05);截趾组(治疗前)血中WBC计数、CRP及PCT水平均高于非截趾组,经比较差异有统计学意义(P<0.05);多因素非条件Logistic相关分析结果显示,肢体缺血、CRP、PCT均为糖尿病足感染患者截趾的独立影响因素。结论血清CRP、PCT水平能有效判断糖尿病足感染的严重程度,有助于临床对于糖尿病足感染患者截趾风险的评估。

OBJECTIVE To evaluate the diagnostic value of serum CRP,PCT and WBC count in diabetic foot infection.METHODS 95patients with diabetic foot infection were grouped according to severity of the disease.Serum levels of CRP,PCT and WBC count were determined before and after treatment.A risk factor analysis was conducted for toe amputation of patients with diabetic foot infection.RESULTS The serum levels of CRP,PCT and WBC count followed the order of severe infection group >moderate infection group >mild infection group,and the difference was significant (P<0.05).The degree of ulceration as well as the incidence of limb ischemia and hypoalbuminemia was significantly higher in the toe amputation group as compared to the non-amputation group (P<0.01).The serum levels of CRP,PCT and WBC count of the toe amputation group before treatment were significantly higher than those in the non-amputation group (P(0.05).The results of multivariate non-conditional Logistic correlation analysis showed that limb ischemia,CRP and PCT were independent risk factors for toe amputation of patients with diabetic foot infection.CONCLUSION The serum levels of CRP and PCT can effectively determine the severity of diabetic foot infection,which can help to evaluate the risk of toe amputation in patients with diabetic foot infection.
