会阴侧切分娩产妇感染病原菌分布及感染途径调查

Distribution of pathogens and pathways of infection in maternal with perineum incision

目的探讨会阴侧切分娩产妇感染的病原菌分布情况及其感染途径。方法选取2014年1月-2016年12月在医院进行会阴侧切分娩的986例产妇作为临床研究对象,并分别对会阴侧切分娩产妇感染的病原菌分布和耐药情况及其感染途径予以全面深入的统计和分析。结果 986例行会阴侧切分娩产妇中有56例发生感染,感染率为5.68%。56例会阴侧切分娩感染产妇中共检出病原菌86株,其中革兰阴性菌52株占60.47%、革兰阳性菌32株占37.21%、真菌2株占2.33%;会阴侧切分娩产妇感染主要革兰阴性菌对氯霉素、庆大霉素、克林霉素、多黏菌素均表现出较高的耐药性,主要革兰阳性菌对头孢硫咪、头孢拉定、环丙沙星均表现出较高的耐药性;会阴侧切分娩产妇感染途径以切口为主,27例占48.21%,其次为泌尿道15例占26.79%。结论在临床工作中应掌握会阴侧切分娩产妇感染的病原菌分布、耐药及感染途径,采取相应的预防措施,合理使用抗菌药物。

OBEJCTIVE:To probe into and analyze the distribution of pathogens and pathways of infection in maternal with perineum incision.METHODS A total of 986 maternal with perineum incision in the hospital from Jan.2014 to Dec.2016 were included in the clinical study.The distribution and drug resistance of pathogens,as well as the pathways of infection in maternal with perineum incision were analyzed in detail.RESULTS In the 986 maternal with perineum incision,there were 56 cases with infection,and the infection rate was 5.68%.A total of 86 strains of pathogens were isolated from the 56 infected maternal with perineum incision,among which 52 strains were gram-negative bacteria accounting for 60.47%,32 strains were gram-positive bacteria accounting for 37.21%,and2 strains were fungi accounting for 2.33%.The main gram-negative bacteria in infected maternal with perineum incision showed relatively high resistance to chloramphenicol,gentamicin,clindamycin and polymyxin.The main gram-positive bacteria showed relatively high resistance to cefotaxime,cefradine and ciprofloxacin.In terms of infection pathway,27 cases of infection occurred at the incision,accounting for 48.21%,followed by 15 cases of urinary tract infection,accounting for 26.79%.CONCLUSION The clinicians should get to know the distribution and drug resistance of pathogens,as well as the infection pathways of infections in maternal with perineal incision,take appropriate preventive measures,and rationally use antibacterial drugs.
