Real-time shear wave elastography in diagnosis of female bladder neck obstruction

摘要 目的探讨实时剪切波弹性成像(SWE)诊断女性膀胱颈梗阻(FBNO)的价值。方法收集27例FBNO患者(FBNO组)和24名健康志愿者(对照组),均接受经会阴超声检查及SWE。观察2组的超声表现,测量并比较2组膀胱颈前唇、后唇厚度及杨氏模量值平均值(E_(mean))和最大值(E_(max))。结果 FBNO组膀胱颈前唇、后唇厚度分别为(0.64±0.08)cm和(0.68±0.09)cm,对照组分别为(0.45±0.07)cm和(0.51±0.07)cm,2组间差异均有统计学意义(t=8.420、7.231,P均<0.001)。FBNO组膀胱颈E_(mean)、E_(max)分别为(27.20±8.63)kPa和(51.20±12.31)kPa,对照组分别为(15.30±3.62)kPa和(29.20±8.73)kPa,2组间差异均有统计学意义(t=6.004、7.407,P均<0.001)。结论 SWE能定量反映膀胱颈硬度,对诊断FBNO有一定价值。 Objective To explore the value of real-time shear wave elastography(SWE)in diagnosis of female bladder neck obstruction(FBNO).Methods Totally 27patients with FBNO(FBNO group)and 24normal women(control group)were enrolled and underwent perineal ultrasound and SWE.The thickness of bladder neck’s anterior lip and posterior lip and the mean and maximum Young’s modulus(Emeanand Emax)were measured and compared between the two groups.Results The average thickness of the anterior and posterior bladder lip FBNO group were(0.64±0.08)cm and(0.68±0.09)cm,in control group of were(0.45±0.07)cm and(0.51±0.07)cm,respectively.There were significant differences between the two groups(t=8.420,7.231,both P<0.001).Emaxand Emeanof FBNO group were(27.20±8.63)kPa and(51.20±12.31)kPa,of control group were(15.30±3.62)kPa and(29.02±8.73)kPa,respectively.There were significant differences between the two groups(t=6.004,7.407,both P<0.001).Conclusion Real-time SWE can reflect the stiffness of the bladder neck quantitatively,therefore having certain diagnostic value for FBNO.
