经腹与经阴道彩色多普勒超声诊断异位妊娠的临床效果分析

Comparative analysis of transabdominal color Doppler ultrasound and transvaginal color Doppler ultrasound in the diagnosis of ectopic pregnancy

目的探讨经腹彩色多普勒超声(TACDS)与经阴道彩色多普勒超声(TVCDS)在异位妊娠(EP)诊断中的临床应用价值。方法回顾性分析安康市妇幼保健院2015年2月至2018年3月收治的80例EP患者的临床资料,按照超声检查方法不同分为TACDS组与TVCDS组各40例,分析两种超声诊断方法检查EP的典型声像图表现,比较两种超声检查方法检出EP的准确率。结果 TVCDS组患者的检出准确率为92.5%,明显著高于TACDS组的70.0%,差异具有统计学意义(P<0.05);TVCDS组患者附件区包块检出率、盆腔积液检出率、子宫内假孕囊检出率、外孕囊影中可见胚芽率、外孕囊影中可见原始心管搏动检出率分别为92.5%、97.5%、47.5%、80.0%、57.5%,均明显高于TACDS组的75.0%、72.5%、12.5%、30.0%、22.5%,差异均具有统计学意义(P<0.05)。结论 TVCDS可极大提高EP的诊断准确率,在EP的诊断方面具有独特的优势,可极大改善EP的早期诊断。

Objective To discuss the clinical value of transabdominal color Doppler ultrasound (TACDS) and transvaginal color Doppler ultrasound (TVCDS) in the diagnosis of ectopic pregnancy (EP). Methods The clinical data of 80 patients with EP who admitted to Ankang Maternal and Child Health Hospital during February 2015 to March 2018 were retrospectively analyzed. According to different ultrasonic examination methods, these patients were divided into the TACDS group (n=40) and TVCDS group (n=40). The accuracy and typical sonographic findings of the two methods were compared. Results The detection accuracy of TVCDS group was 92.5%, which was significantly higher than 70.0% of the TACDS group (P<0.05). The detection rate of adnexal mass, pelvic effusion, intrauterine pseudocyst, germ in ectopic cyst shadow and primitive cardiac pulsation in ectopic cyst shadow in the TVCDS group were 92.5%, 97.5%, 47.5%, 80.0%, 57.5%, respectively, which were significantly higher than corresponding 75.0%, 72.5%, 12.5%, 30.0%, 22.5% in the TACDS group (P<0.05). Conclusion TVCDS has a unique advantage in the diagnosis of EP and can greatly improve the early diagnosis of EP.
