社区医务人员压力现状调查与分析

Investigation and analysis of community medical staff's pressure status

目的由于医务人员工作的特殊性,所面临的工作形势和承担的工作强度使其产生心理压力。本研究旨在了解目前基层医务人员的压力现状,为提高基层医务工作者工作满意度和缓解执业倦怠提供科学理论依据。方法采用知觉压力量表(Chinese-14-item perceptual pressure scale,CPSS)于2017-07-01—2017-09-30对长宁区北新泾街道社区卫生服务中心、新泾镇社区卫生服务中心和仙霞街道社区卫生服务中心328名基层医务人员进行基本信息及知觉压力问卷调查,并分析基层医务人员压力现状。结果有效问卷328份,有效回收率为92. 1%。208名(63. 4%)基层医务人员知觉压力量表得分≥25分,处于健康风险压力状态。岗位为工作室的医务人员知觉压力得分最高(27. 53±7. 08)分,其次是管理岗位(26. 13±6. 17)分,F=3. 218,P=0. 018;在岗时间> 5～10年得分最高(27. 72±5. 18)分,其次是> 10～15年得分(26. 68±4. 23)分,> 20年得分最低(19. 98±4. 92)分,F=2. 338,P=0. 039;执业范围中医生得分最高(26. 52±6.28)分,其次是行政管理得分(25. 82±5. 87)分,医技得分最低(19. 92±5. 81)分,F=3. 318,P=0. 016。结论基层医务人员工作压力较大,值得社会及医疗卫生管理者高度的重视与关注,应当从社会、医院和个人等层面及早采取相应的措施,缓解医务工作者的身心压力。

OBJECTIVE Because of the particularity of their work,the medical staff usually suffer from psychological pressure produced by working situation and the intensity of the work.The purpose of this study is to understand the current stress situation of grass-roots medical staff,and to provide scientific theoretical basis for improving job satisfaction and alleviating job burnout of grass-roots medical workers.METHODS The basic information and perceived stress of 328grass-roots medical staff in Beixinjing Street Community Health Service Center,Xinjing Town Community Health Service Center and Xianxia Street Community Health Service Center in Changning District were investigated with the Chinese-14-item perceptual pressure scale(CPSS)from July 1 to September 30in 2017,and the pressure status on grass-roots medical staff were analyzed.RESULTS There were 328valid questionnaires,with an effective recovery rate of 92.1%. The CPSS scores of 63.4% grass-roots medical staff were more than 25 scores,at a health risk stress state.The highest score of perceived stress was for the medical staff working in the studio (27.53±7.08)score,followed by management position (26.13±6.17)score,F=3.218,P =0.018.The score of the staff on duty for >5-10years was highest (27.72±5.18),followed by (26.68±4.23) of those on duty for >10-15 years,the lowest score was (19.98±4.92)of those on duty for >20 years,F =2.338,P =0.039.The highest score of practicing doctors was (26.52±6.28)score, followed by (25.82±5.87)scores in administration and (19.92±5.81)scores in medical skills,F=3.318,P=0.016. CONCLUSIONS The pressure of grass-roots medical staff is great,which deserves the high attention of the society and medical and health administrators.The corresponding measures should be taken as soon as possible at the social,hospital and individual levels to alleviate the physical and mental pressure of medical workers.
