胃癌化疗患者全面优质护理方案效果量表评分分析

Analysis of the effect scale of comprehensive quality nursing program for gastric cancer patients with chemotherapy

目的胃癌在我国恶性肿瘤发病率中居于首位,严重威胁着人类的生命健康。化疗是治疗中晚期胃癌患者的重要手段,但严重的化疗不良反应不仅会影响患者的治疗效果,还会降低患者的生活质量。本研究探讨全面优质护理模式对胃癌化疗患者不良情绪以及对评分的影响,为临床胃癌化疗患者的治疗和护理提供指导。方法选择山东省肿瘤医院2016-01-01-2017-01-01收治的148例胃癌化疗患者为研究对象,将其随机分为常规护理组和全面护理组,各74例,分别采用常规护理和全面优质护理,护理后评价两组疗效,采用自我护理能力量表(self-care ability scale for the elderly,ESCA)和癌症患者生命质量测定量表(quality of life instruments for cancer patients,QLICP)对患者的自我护理能力和生命质量进行评分,采用抑郁焦虑评分表对患者的不良情绪进行评价,最后评价患者的满意度。结果全面护理组临床有效率为60.81%,明显优于常规护理组的36.49%,差异有统计学意义,χ2=19.475,P<0.001。护理1个月后,两组ESCA评分和QLICP评分明显升高,全面护理组ESCA评分为(48.01±4.98)分,高于常规护理组的(41.83±4.24)分,t=9.382,P<0.001;全面护理组QLICP评分为(70.38±5.32)分,高于常规护理组的(62.10±5.21)分,t=8.107,P<0.001;护理1个月后,两组SAS和SDS评分均有明显下降,全面护理组SAS评分为(33.14±3.72)分,低于常规护理组的(43.99±3.64)分,t=11.127,P=0.002;全面护理组SDS评分为(35.47±2.97)分,低于常规护理组的(45.46±3.45)分,t=6.895,P<0.001。常规护理组满意度为89.19%,明显低于全面护理组的97.30%,差异有统计学意义,χ2=6.66,P=0.010。结论全面优质护理应用于胃癌化疗患者,护理效果显著,能明显提高患者生活质量和自我护理能力,缓解患者不良情绪。

OBJECTIVE Gastric cancer ranks first in the incidence of malignant tumors in China,which seriously threatens human life and health.Chemotherapy is an important means in the treatment of advanced gastric cancer patients,but serious adverse reactions of chemotherapy will not only affect the efficacy of patients,but also reduce the life quality of patients.This study aims to explore the effects of comprehensive quality nursing model on adverse mood,self-care ability scale for the elderly(ESCA)and quality of life instruments for cancer patients(QLICP)scores of patients with gastric cancer chemotherapy,to provide guidance for the treatment and nursing of gastric cancer patients with clinical chemotherapy.METHODS 148 cases of patients with gastric cancer chemotherapy in our hospital were selected from January 1,2016 to January 1,2017 as the research subjects,divided into two groups according to random number table method,with 75 cases in each group.One group of patients adopt the routine nursing and the other treated with comprehensive quality nursing mode.ESCA and QLICP scale were used to evaluate the self-care ability and life quality effects of patients in two groups after nursing.Depression and anxiety scale were used to evaluate the feelings of the patients.Finally the patients’ satisfaction were evaluated.RESULTS The clinical effective rate of comprehensive nursing group was 60.81%,which was obviously better than that of routine nursing group(36.49%)with a statistically significant difference,χ2=19.475,P<0.001.After one month of nursing,the ESCA score and QLICP score of the two groups increased significantly.The ESCA score of the comprehensive nursing group was(48.01±4.98),higher than that of the routine nursing group(41.83±4.24),t=9.382,P<0.001.The QLICP score of the comprehensive nursing group was(70.38±5.32),higher than that of the routine nursing group(62.10±5.21),t=8.107,P<0.001.After one month of nursing,the scores of SAS and SDS in both groups were significantly decreased.The score of SAS in the comprehensive nursing group was(33.14±3.72),lower than that in the routine nursing group(43.99±3.64),t=11.127,P=0.002.And the score of SDS in the comprehensive nursing group was(35.47±2.97),lower than that in the routine nursing group(45.46±3.45),t=6.895,P<0.001.The satisfaction rate of routine nursing group was 89.19%,which was significantly lower than 97.30%of total nursing group,χ2=6.66,P=0.010.CONCLUSIONS The comprehensive high-quality nursing has significant effect in patients with gastric cancer chemotherapy.It can significantly improve the life quality and self-care ability of patients,as well as alleviate patients’ adverse mood.
