健脾益肾方对妊娠期糖尿病患者妊娠结局的影响

Effect of Jianpi Yishen Prescription on Pregnancy Outcomes in Patients with Gestational Diabetes

目的:观察健脾益肾方联合综合疗法对妊娠期糖尿病患者妊娠结局的影响。方法:126例妊娠期糖尿病患者随机分为观察组(63例)和对照组(63例)。对照组给予西医常规疗法,观察组在对照组治疗基础上给予健脾益肾方。比较两组患者临床疗效,中医证候积分,血脂、血糖水平,妊娠结局。结果:治疗前两组患者中医证候积分比较,差异无统计学意义(P> 0. 05),治疗后两组患者评分均显著降低(P <0. 05),观察组显著低于对照组(P <0. 05)。观察组治疗有效率为93. 65%,对照组为76. 19%,两组有效率比较,差异有统计学意义(P <0. 05)。治疗前两组患者空腹血糖、糖化血红蛋白、总胆固醇水平比较,差异无统计学意义(P> 0. 05);治疗后两组患者以上指标水平均显著降低(P <0. 05),观察组显著低于对照组(P <0. 05)。观察组母体发生胎膜早破、羊水过多、早产、剖宫产,围生儿出现新生儿窒息、巨大儿、胎儿宫内窘迫概率显著低于对照组,差异有统计学意义(P <0. 05)。结论:健脾益肾方可显著缓解妊娠期糖尿病患者临床症状,降低中医证候积分和血糖、血脂水平,改善妊娠结局。

Objective: To observe the effect of Jianpi Yishen Prescription combined with comprehensive therapy on pregnancy outcomes in patients with gestational diabetes. Methods: 126 patients with gestational diabetes were randomly divided into observation group ( 63 cases) and control group ( 63 cases) . The control group was given conventional western medicine therapy,and the observation group was given Jianpi Yishen prescription on the basis of the control group. The clinical efficacy of the two groups of patients, TCM syndrome scores,blood lipids,blood glucose levels,pregnancy outcomes were compared. Results: There was no significant difference in TCM syndrome scores between the two groups before treatment ( P > 0. 05) . After treatment,the scores of the two groups were significantly lower ( P < 0. 05) , and the observation group was significantly lower than the control group ( P < 0. 05) . The effective rate of treatment was 93. 65% in the observation group and 76. 19% in the control group. The effective rate of the two groups was statistically significant ( P < 0. 05) . There was no significant difference in fasting blood glucose,glycosylated hemoglobin and total cholesterol between the two groups before treatment ( P > 0. 05) . After treatment,the above indexes of the two groups were significantly lower ( P < 0. 05) , and the observation group was significantly lower than the control group ( P < 0. 05) . The maternal premature rupture of membranes,polyhydramnios,premature delivery,cesarean section,the incidence of neonatal asphyxia, macrosomia,and intrauterine distress in the perinatal group were significantly lower than that in the control group,and the differences were statistically significant ( P < 0. 05) . The probability of occurrence of premature rupture of membranes,excessive amniotic fluid,premature delivery,cesarean section,neonatal asphyxia in perinatal,macrosomia,probability of intrauterine distress in the observation group was significantly lower than that in the control group,and the difference was statistically significant ( P < 0. 05) . Conclusion: Jianpi Yishen Prescription can significantly alleviate the clinical symptoms of gestational diabetes patients,re- duce TCM syndrome scores,blood sugar and blood lipid levels,and improve pregnancy outcomes.
