实时超声弹性成像诊断甲状腺单发实性小结节的价值分析

摘要 目的分析实时超声弹性成像诊断甲状腺单发实性小结节的临床价值。方法对民权县人民医院在2016年10月—2017年12月期间所收治的118例甲状腺单发实性小结节患者进行研究,对其进行灰阶超声检查,观察结节大小、边界以及内部回声等,随后用彩色能量多普勒超声观察结节内有无彩色血流分布,接着对纵切面进行实时弹性成像,将其分为1~5分,对比分析超声诊断结果与病理结果。结果超声弹性成像结果表明:1分与2分有29例患者,28例为良性,1例恶性;36例为3分,其中包括28例良性,8例恶性;其余53例为4分、5分患者,包括8例良性与45例恶性。经病理分析,118例均为单发实性低回声病灶,其中良性患者64例,恶性患者54例,病理诊断结果与实时超声弹性成像结果基本相符。结论实时超声弹性成像应用在甲状腺单发实性小结节鉴别诊断中的效果明显,值得推广应用。 Objective To analyze the clinical value of real-time ultrasound elasticity imaging in the diagnosis of thyroid simple nodules. Methods A total of 118 patients with simple thyroid nodules were studied in our hospital from October2016 to December 2017. The gray scale ultrasonography was performed to observe the nodule size,boundary and internal echo. Followed by color Doppler ultrasonography to observe the distribution of color flow within the nodules,followed by real-time elastic imaging of the longitudinal section,divided into 1-5 grade,comparative analysis of ultrasound diagnosis and pathological results. Results Ultrasound elastic imaging results showed that: 1 point and 2 points in 29 patients,including 28 cases of benign,1 cases of malignant; 36 cases of 3 points,including 28 cases of benign,8 cases of malignant; the remaining 53 cases patients of 4 points and 5 points,including 45 cases of vicious and 8 cases of benign. Pathological analysis,118 cases were single real hypoechoic lesions,of which 64 cases of benign patients,54 cases of malignant patients,pathological diagnosis and real-time ultrasound elastic imaging results are similar.Conclusion Real-time ultrasound elasticity imaging is effective in the differential diagnosis of thyroid simple nodules.It is worthy to be popularized and applied.
