P16和Ki-67表达量在宫颈癌早期诊断及癌细胞恶性行为评估中的作用 被引量:7

Significance of the expression of P16 and Ki-67 genes in early diagnosis of cervical carcinoma and assessment of malignant behavior

摘要 目的探讨宫颈癌病灶中P16和Ki-67基因表达量在宫颈癌早期诊断及癌细胞恶性行为评估中的作用。方法选取2015年12月至2018年3月间陕西省肿瘤医院收治的行阴道镜下活检检查的189例宫颈病变患者,根据病理结果分组,宫颈息肉组(71例),宫颈上皮内瘤变组(40例),宫颈癌组(78例)。比较三组患者宫颈病灶组织中P16、Ki-67基因、EMT相关基因和自噬相关基因的表达量,分析宫颈癌病灶组织中P16和Ki-67基因表达量与癌细胞上皮间质转化(EMT)和自噬活性的相关关系。结果宫颈上皮内瘤变组的P16和Ki-67基因表达量高于宫颈息肉组,EMT相关基因及自噬相关基因表达量低于宫颈息肉组,差异均有统计学意义(均P <0. 05)。宫颈上皮内瘤变组的P16和Ki-67及EMT相关基因Snail和Twist基因表达量高于宫颈上皮内瘤变组和宫颈息肉组,EMT相关基因E-cadherin和vimentin及自噬相关基因表达量低于宫颈瘤组和宫颈息肉组,差异均有统计学意义(均P <0. 05)。宫颈癌患者病灶组织中P16和Ki-67表达量与EMT相关基因E-cadherin和vimentin的表达量呈负相关,与EMT相关基因Snail和Twist的表达量呈正相关,差异均有统计学意义(均P <0. 05)。宫颈癌患者病灶组织中P16和Ki-67表达量与自噬相关基因Beclin1、MAPLC3和ATG2B的表达量呈负相关,差异均有统计学意义(均P <0. 05)。结论宫颈癌组织中P16和Ki-67基因表达量异常增加,且具体表达量与癌细胞EMT及自噬活力直接相关,可成为日后宫颈癌治疗的新靶点。 Objective To detect the significance of the expression of P16 and Ki-67 genes in early diagnosis of cervical carcinoma and assessment of malignant behavior. Methods A total of 189 patients with cervical lesions undergoing vaginoscopy at Shaanxi Province Tumor Hospital from December 2015 to March 2018 were selected as the research subjects. According to pathological results,the patients were divided into cervical polyps group( 71 patients),cervical intraepithelial neoplasia group( 40 patients) and cervical cancer group( 78 patients). The expressions of P16 and Ki-67 genes,epithelial-mesenchymal transition( EMT) related genes and autophagy related genes in cervical lesions were compared among the three groups,and the correlation between the expressions of P16 and Ki-67 genes and EMT and autophagy activity was analyzed. Results The expression of P16 and Ki-67 genes was significantly higher in the cervical intraepithelial neoplasia group than in the cervical polyps group,and the expression of EMT related genes and autophagy related genes in cervical lesions was lower than in the cervical polyps group( all P<0. 05). The expression of P16 and Ki-67 and EMT related genes Snail and Twist was significantly higher in the cervical cancer group than other two groups and the expression of EMT related genes E-cadherin and vimentin was significantly lower than other two groups( all P<0. 05). The expressions of P16 and Ki-67 genes in cervical cancer tissues were positively correlated with EMT related genes E-cadherin and vimentin,and negatively correlated with a EMT related genes Snail 和 Twist( all P<0. 05). The expressions of P16 and Ki-67 genes in cervical cancer tissues were negatively correlated with autophagy related genes Beclin1,MAPLC3 and ATG2 B. Conclusion The expression of P16 and Ki-67 genes in cervical cancer tissues increase abnormally,and the specific expressions are directly related to EMT and autophagy activity of cancer cells,which may become new targets for the treatment of cervical cancer in the future.
