股前外侧皮瓣修复术对口腔肿瘤患者术后生活质量的影响

Effect of anterior thigh flap reconstruction on the postoperative quality of life in patients with oral tumors

目的探讨股前外侧皮瓣修复术对口腔肿瘤患者术后生活质量的影响。方法选取2013年1月至2016年12月间空军军医大学口腔医院收治的83例口腔肿瘤切除术后组织缺损患者,根据术后修复方式不同分为股前外侧皮瓣修复组42例和前臂皮瓣修复组41例。比较两组患者术后皮瓣存活率、术后6个月饮食功能恢复情况、受区外形、功能障碍及生存质量。结果术后,股前外侧皮瓣修复组皮瓣存活率、术后6个月饮食功能恢复情况、受区外形及生存质量与前臂皮瓣修复组相当,但功能障碍发生率低于前臂皮瓣组,差异有统计学意义(P <0. 05)。结论口腔肿瘤术后组织缺损者可首选股前外侧皮瓣修复术,但需在术者熟练操作下提高皮瓣存活率。

Objective To investigate the effect of anterior thigh flap reconstruction on the postoperative quality of life in patients with oral tumors. Methods A total of 83 patients with tissue defect after oral tumor resection at Air Force Military Medical University School of Stomatology from January 2013 to December 2016 were selected. They were divided into the anterior thigh flap reconstruction group( 42 patients)and forearm flap reconstruction group( 41 patients). Postoperative flap survival,diet function recovery,shape of the affected area,dysfunction and quality of life at 6 months after the operation were compared between the two groups. Results The postoperative flap survival,diet function recovery,area shape and quality of life at 6 months after the surgery were comparable between the two groups,while the incidence of dysfunction in the anterior thigh flap reconstruction group than in the forearm flap reconstruction group( P <0. 05). Conclusion When conditions permit,the anterior thigh flap reconstruction was the first choice for patients with tissue defect complicated with oral tumor,while it should be performed by experienced surgeon to improve the survival.
