高危型HPV感染与宫颈癌患者病理特征和Th17/Treg免疫应答的相关性研究 被引量:16

摘要 目的探讨高危型人乳头瘤病毒(HPV)感染与宫颈癌患者病理特征和T辅助细胞17(Th17)/调节性T细胞(Treg)免疫应答的相关性研究。方法选取2014年1月至2017年6月间陕西省康复医院收治的行手术治疗的90例宫颈癌患者,测定手术切除的宫颈癌病灶内高危型HPV的感染情况、血清中肿瘤标志物及Th17/Treg细胞因子含量。结果宫颈癌患者国际妇产科协会Ⅱ期病灶内高危型HPV感染率高于Ⅰ期,病理分级G3宫颈癌病灶内高危型HPV感染率高于病理分级G1+G2,差异均有统计学意义(均P <0. 05)。高危型HPV阳性宫颈癌患者血清中,糖类抗原125、鳞状细胞癌抗原、肿瘤特异性生长因子和细胞角蛋白19片段抗原含量均高于高危型HPV阴性的宫颈癌患者,差异均有统计学意义(均P <0. 05)。高危型HPV阳性的宫颈癌患者血清中白介素10、白介素17、白介素23和转化生长因子-β1含量,均高于高危型HPV阴性的宫颈癌患者,差异均有统计学意义(均P <0. 05)。结论高危型HPV感染能促进宫颈癌患者临床分期及病理分级的进展,增加肿瘤标志物分泌,破坏Th17/Treg免疫应答平衡。 Objective To study the relationship between high-risk human papillomavirus( HPV)infection and pathological features of cervical cancer and T helper cell 17( Th17)/regulatory T cell( Treg)immune response. Methods Ninety patients with cervical cancer who underwent surgical treatment at Shaanxi Provincial Rehabilitation Hospital from January 2014 to June 2017 were selected as the subjects.The infection of high-risk HPV in the cervical cancer lesions,and the levels of tumor markers and Th17/Treg cytokines in serum were measured. Results The infection rate of high-risk HPV in International Federation of Gynecology and Obstetrics( FIGO) stage II cervical cancer was higher than in FIGO stage I cervical cancer,and the infection rate of high-risk HPV in pathological grade G3 cervical cancer was higher than in pathological grade G1 and G2 cervical cancer( all P < 0. 05). The levels of serum carbohydrate antigen125( CA125),squamous cell carcinoma antigen( SCC-Ag),tumor specific growth factor( TSGF),cytokeratin-19-fragment( Cyfra211),interleukin( IL)-10,IL-17,IL-23 and transforming growth factor( TGF)-β1 in cervical cancer patients with positive HPV infection were higher than in patients with negative HPV infection( all P < 0. 05). Conclusion High-risk HPV infection can promote the progression of clinical stage and pathological grade of cervical cancer,increase the secretion of tumor markers and destroy the balance of Th17/Treg immune response.
