原发性开角型青光眼合并2型糖尿病患者眼底改变、血管内皮功能及血液流变学指标检测及意义

Detection and significance of ocular fundus change,vascular endothelial function and hemorheological indexes in patients with type 2 diabetes mellitus and primary open angle glaucoma

目的探讨原发性开角型青光眼(primary open-angle glaucoma,POAG)合并2型糖尿病患者眼底改变、血管内皮功能及血液流变学指标水平变化及意义。方法120例POAG患者依据是否合并2型糖尿病分为单纯POAG组(65例)和合并糖尿病组(55例),另选取同期健康体检者50例作为对照组,比较3组间受试者的眼底改变、血管内皮功能及血液流变学指标水平。结果与对照组相比,单纯POAG组及合并糖尿病组的眼压水平均显著升高(P<0.05),RNFL及MD水平均显著降低(P<0.05);单纯POAG组及合并糖尿病组ET-1水平显著升高(P<0.05),NO水平显著降低(P<0.05);单纯POAG组及合并糖尿病组FIB、高切全血黏度和低切全血黏度水平均升高(P<0.05);合并糖尿病组患者FIB、高切全血黏度和低切全血黏度水平高于单纯POAG组和对照组,上述差异均有统计学意义(P<0.05)。结论POAG合并2型糖尿病患者存在明显眼底改变、血管内皮功能受损及血液流变学异常,对病情程度评估具有重要价值。

Objective To investigate the changes and significance of fundus changes,vascular endothelial function and hemorheological indexes levels in patients with primary open -angle glaucoma (POAG)combined with type 2 diabetes. Methods Totally 120 cases of POAG patients were divided into pure POAG group and POAG combining diabetes group according to whether they suffered from type 2 diabetes.At the same time,50 healthy subjects were selected as control group.The fundus changes,endothelial function and hemorheological indexes of the three groups were compared.Results Compared with the control group,the levels of intra -ocular tension in pure POAG group and POAG combining diabetes group increased,while RNFL and MD levels decreased (P <0.05);the ET -1 levels in POAG group and POAG combining diabetes group significantly increased (P <0.05),while the NO level significantly decreased (P <0.05).FIB,high-cut whole blood viscosity and low - cut whole blood viscosity increased in the POAG group and the POAG combining diabetes group (P <0.05),while that of POAG combining diabetes group were higher than those of POAG group and control group,with the differences statistically significant (P <0.05).Conclusion Patients with POAG combined with type 2 diabetes have obvious fundus change,vascular endothelial dysfunction and abnormal hemorheology,which is of great value for.evaluating disease severity.
