炎症诊断试验在儿童细菌感染中的应用 被引量:4

Application of inflammatory diagnostic tests in children’s bacterial infections

摘要 目的探讨血降钙素原(PCT)与超敏C-反应蛋白(hs-CRP)联合测定在儿科患者的细菌感染诊断中的应用价值。方法选取2016年9月-2017年4月本院确诊治疗的感染患儿124例,将其分为细菌感染组64例和非细菌感染组60例,所有患儿检测血清PCT、hs—CRP,分析所有患儿血清PCT、hs—CRP水平,采用ROC曲线分析单独PCT、hs-CRP及二者联合测定诊断患儿细菌感染的准确性。结果细菌感染组PCT、hs—CRP水平均高于非细菌感染组和对照组(P<0.05);重症感染组PCT、hs-CRP水平均高于轻症感染组(P<0.05)。ROC曲线下面积显示,在患儿细菌感染诊断价值方面,PCT、hs-CRP联合测定优于二者单独测定。结论血清PCT、hs-CRP作为严重细菌性炎症的特异性和早期诊断指标,两者联合测定对患儿细菌感染具有更高的诊断价值,可作为脓毒症和炎症活动有关的多脏器衰竭的可靠指标。 Objective To evaluate the application of serum procalcitonin( PCT) and hypersensitive C-reactive protein( hs-CRP) in the diagnosis of bacterial infection in pediatric patients. Methods A total of 124 children with infection diagnosed in our hospital were selected from September 2016 to April 2017 and divided into bacterial infection group( 64 cases)and non-bacterial infection group( 60 cases). PCT and hs-CRP levels were measured in all children. ROC curve was used to analyze the efficacy of PCT or hs-CRP separately detection and the efficacy of PCT combined with hs-CRP in the diagnosis of bacterial infection. Results The levels of PCT and hs-CRP in children with bacterial infection were significantly higher than those in non-bacterial group,and the differences were statistically significant( P < 0. 05). The area under ROC curve showed that the combined determination of PCT and hs-CRP in diagnosis value of bacterial infection was better than that of separate determination. Conclusion As the specific and early diagnostic indicators of severe bacterial inflammation,the combined determination of PCT and hs-CRP in children with bacterial infection has higher diagnosis value,which can be used as reliable index for sepsis and inflammation activities related multiple organ failure.
