2017年温州瓯海区食源性腹泻病例流行特征及其影响因素分析

Analysis of epidemic character and influence factors of food-borne diarrhea in Ouhai District in 2017

目的了解瓯海区食源性腹泻病例的流行特征及其影响因素,为防控腹泻病提供科学依据。方法收集瓯海区2017年1家食源性疾病监测哨点医院的监测数据共计514例,标本进行病原学检测。描述检出病原的腹泻病例的流行特征,并采用非条件Logistic回归分析病原感染的影响因素。结果监测病例中诺如病毒阳性率为8.4%(43/514),以GⅡ型为主,冬春季高发,副溶血性弧菌阳性率为13.8%(71/514),以O3群为主,秋季高发。多因素分析结果显示,性别、年龄、职业、季节是食源性疾病监测病例诺如病毒阳性检出的影响因素(P<0.05),季节是食源性疾病监测病例副溶血性弧菌阳性检出的影响因素(P<0.05)。结论副溶血性弧菌和诺如病毒是本地区食源性腹泻的主要致病原因,结合病原学影响因素展开针对性的防控,对于控制本地区食源性腹泻疫情有重要意义。

Objective To study the epidemic character and influence factors of food-borne diarrhea in Ouhai District in 2017,so as to provide scientific basis to control the food-borne diarrhea in this area. Methods Totally 514 cases of monitoring data from 1 sentinel hospital of Ouhai were collected for pathogen detection. Specimens were conducted for pathogen detection. To describe the epidemiological characteristics of diarrhea cases in which pathogens were detected,and use unconditional Logistic regression to analyze the influencing factors of pathogen infection. Results The positive rate of nororvirus was 8. 4%( 43/514). Nororvirus were detected mainly in type G Ⅱ in spring and winter. The positive rate of Parahaemolyticus was13. 8%( 71/514),Parahaemolyticus was detected mainly in O3 group in fall. The result of the multivariate factor analysis showed that genders,ages,occupations and seasons were the risk factors of nororvirus infection( P < 0. 05). Seasons were the risk factors of Parahaemolyticus infection( P < 0. 05). Conclusion Parahaemolyticus and norovirus infection are the main reasons of diarrhea in Ouhai. Combination of pathogen factors education is very important for controlling the food-borne diarrhea.
