对在ICU接受机械通气治疗的老年重症肺炎患者实施综合护理干预的效果评价

Evaluation of the effect of comprehensive nursing intervention on elderly patients with severe pneumonia receiving mechanical ventilation in ICU

目的分析对在重症监护病房(ICU)接受机械通气治疗的老年重症肺炎患者实施综合护理干预的效果。方法 76例在ICU接受机械通气治疗的老年重症肺炎患者,按照简单随机法分成对照组与观察组,各38例。对照组采用常规护理方案,观察组采用综合护理干预,对比两组护理效果。结果干预后,观察组机械通气时间、住院时间分别为(4.12±1.44)、(14.95±2.30)d,显著短于对照组的(5.63±1.83)、(18.45±3.19)d,差异具有统计学意义(P<0.05);观察组急性生理学和慢性健康状况评分系统Ⅱ(APACHEⅡ)、氧合指数、每分钟通气量分别为(23.57±6.48)分、(167.85±22.84)mm Hg(1 mm Hg=0.133 kPa)、(8.84±2.81)L,均优于对照组的(30.76±5.95)分、(154.65±20.23)mm Hg、(6.25±1.93)L,差异具有统计学意义(P<0.05)。结论在ICU接受机械通气的老年重症肺炎患者经综合护理干预措施,能显著改善其临床表现,治疗效果较好,值得临床进一步推广应用。

Objective To analyze the effect of comprehensive nursing intervention on elderly patients with severe pneumonia receiving mechanical ventilation in intensive care unit (ICU). Methods A total of 76 elderly patients with severe pneumonia receiving mechanical ventilation in ICU were divided by simple random method into control group and observation group, with 38 cases in each group. The control group received conventional nursing regimen, and the observation group received comprehensive nursing intervention. The nursing effect in two groups was compared. Results After intervention, the observation group had mechanical ventilation time and hospitalization time respectively as (4.12±1.44) and (14.95±2.30) d, which were significantly shorter than (5.63±1.83) and (18.45±3.19) d in the control group, and their difference was statistically significant (P<0.05). The observation group had acute physiology and chronic health evaluation Ⅱ (APACHEⅡ), oxygenation index, minute ventilation volume respectively as (23.57±6.48) points, (167.85±22.84) mm Hg(1 mm Hg=0.133 kPa) and (8.84±2.81) L/min, which were all better than (30.76±5.95) points, (154.65±20.23) mm Hg and (6.25± 1.93) L/min in the control group, and their difference was statistically significant (P<0.05). Conclusion Application of comprehensive nursing intervention shows good effect in treating elderly patients with severe pneumonia receiving mechanical ventilation in ICU , and it can significantly improve their clinical manifestation. It is worthy of clinical promotion and application.
