布地奈德混悬液雾化吸入治疗在经鼻全麻气管插管手术后的临床效果观察

Clinical observation of Budesonide suspension for Inhalation atomization therapy after nasotracheal intubation general anesthesia operation

目的观察布地奈德混悬液雾化吸入治疗在经鼻全麻气管插管手术后的临床疗效。方法选取2017年2月-2018年4月医院经鼻全麻气管插管手术患者100例,随机分为研究组和对照组各50例,研究组拔管后给予布地奈德混悬液雾化吸入,对照组拔管后给予地塞米松雾化吸入。比较2组术后咽痛、声嘶、呼吸困难、咳嗽咯痰的发生率、术后咽喉部黏膜反应评分及雾化前后血氧相关指标。结果研究组术后咽痛、声嘶、呼吸困难及咳嗽咯痰的发生率均低于对照组,差异均有统计学意义(P <0. 05)。研究组术后1 d、术后3 d及术后5 d咽喉部黏膜反应评分均低于对照组,差异均有统计学意义(P <0. 05)。2组雾化前PaO_2、PaCO_2、SpO_2比较差异均无统计学意义(P>0. 05);雾化后2组PaO_2、PaCO_2、SpO_2指标均改善,且研究组改善程度优于对照组,差异均有统计学意义(P <0. 05)。结论经鼻全麻气管插管手术后给予患者布地奈德混悬液雾化吸入治疗,可改善患者喉部不适症状,提高血氧指标,促进咽喉部黏膜屏障的修复。

Objective To observe the clinical efficacy of budesonide suspension nebulization in the treatment of nasotracheal intubation general anesthesia operation. Methods 100 patients after nasotracheal intubation general anesthesia operation were selected from February 2017 to April 2018,and they were randomly divided into study group(50 cases)and control group(50 cases),after the extubation,the study group was given a budesonide suspension for aerosol inhalation,the control group was given dexamethasone inhalation. The incidence of sore throat,hoarseness,dyspnea,cough and cough,postoperative throat mucosal response score and blood oxygenation related indexes before and after nebulization were compared between the two groups. Results The incidence of sore throat,hoarseness,dyspnea,cough and expectoration of the study group was lower than that of the control group,and the differences were statistically significant (P < 0. 05). The scores of throat mucosal response in the study group were lower than that in the control group at 1 d,3 d and 5 d after surgery (P <0. 05). There were no significant differences in PaO 2 ,PaCO 2 and SpO 2 between the two groups before nebulization (P >0. 05). After the atomization, the indicators of PaO 2 ,PaCO 2 and SpO 2 were improved,the improvement of the study group was better than that of the control group,and the differences were statistically significant (P < 0. 05). Conclusion The nebulization of budesonide suspension after nasal general anesthesia can improve the symptoms of discomfort in the larynx,improve the blood oxygen index,and promote the repair of the mucosa barrier of the throat.
