围绝经期激素补充疗法依从性影响因素分析

Analysis of factors influencing the compliance of peri-menopausal hormone replacement therapy

目的分析围绝经期激素补充疗法依从性的影响因素。方法选择围绝经期实施激素补充疗法治疗的患者79例,比较不同年龄、居住地、接受教育时间、经济状况、婚姻、围绝经期及对激素补充疗法认知、心理反应、家庭支持、医疗费用、激素补充疗法不良反应患者的依从性。结果 79例患者依从性好50例(63. 29%),依从性差29例(36. 71%);单因素检验:年龄、居住地、接受教育时间、经济状况、婚姻、病程、围绝经期及对激素补充疗法认知、心理反应、家庭支持、医疗费用、激素补充疗法不良反应比较差异有统计学意义(P <0. 05)。Logistic回归分析:影响激素补充疗法依从性的危险因素为围绝经期及对激素补充疗法认知不足(OR=4. 43,95%CI:1. 26-23. 67)、激素补充疗法不良反应(OR=4. 04,95%CI:1. 36-24. 17)、心理反应不良(OR=3. 53,95%CI:1. 40-20. 18)、经济情况差(OR=2. 76,95%CI:1. 10-18. 37)。结论围绝经期激素补充疗法依从性较差,影响因素复杂,应采取相应措施提高围绝经期激素补充疗法依从性。

Objective To analyze the influencing factors of peri-menopausal hormone replacement therapy compliance. Methods 79 patients with perimenopausal hormone replacement therapy were selected,the compliance of patients with different ages,places of residence,time of education,economic status,marriage,peri-menopausal and hormone-replenishing therapy,psychological response,family support,medical expenses,and adverse reactions to hormone replacement therapy were compared. Results Among the 79 patients,50 patients (63. 29%) had good compliance,and 29 patients (36. 71%) had poor compliance;Univariate test:age,place of residence,time of education,economic status,marriage,duration of illness,perimenopausal and hormonal supplement therapy cognition,psychological reaction,family support,medical expenses,and adverse effects of hormone replacement therapy were statistically significant(P <0. 05). Logistic regression analysis:risk factors for adherence to hormone replacement therapy were cognitive impairment in peri-menopausal and hormone-supplemented therapy (OR =4. 43,95%CI:1. 26 -23. 67),adverse effects of hormone replacement therapy(OR = 4. 04,95% CI:1. 36 - 24. 17), poor psychological reaction (OR = 3. 53,95% CI:1. 40 - 20. 18),poor economic situation (OR = 2. 76,95% CI:1. 10- 38. 37). Conclusion Perimenopausal hormone replacement therapy has poor compliance and complex influencing factors. Corresponding measures should be taken to improve compliance of hormone replacement therapy in perimenopausal period.
