血清总铁结合力检测对慢性肾功能不全诊断的临床价值 被引量:2

摘要 目的探讨血清总铁结合力(TIBC)对慢性肾功能不全诊断的临床价值。方法选取30例慢性肾功能不全患者作为实验组与30例健康人作为对照组,分析两组血清TIBC的水平;ROC曲线评估TIBC对慢性肾功能不全诊断的价值;Kappa检验分析TIBC对慢性肾功能不全诊断与肌酐诊断结果的一致性。结果慢性肾功能不全组的血清TIBC为(40.86±7.54)μmol/L,而对照组血清TIBC为(61.96±7.48)μmol/L,差异有统计学意义(t=10.87,P<0.05);TIBC诊断慢性肾功能不全的ROC曲线下面积为0.979,血清TIBC取截断值为49μmol/L时,诊断敏感度为86.7%、特异度为100%;TIBC对慢性肾功能不全诊断与肌酐诊断具有良好的一致性(Kappa=0.867,P<0.05)。结论血清总铁结合力(TIBC)对慢性肾功能不全的诊断具有较高的诊断效能,可以用于慢性肾功能不全的辅助诊断。 Objective To evaluate the clinical value of serum total iron binding capacity (TIBC)in the diagnosis of chronic renal failure.Methods 30cases of chronic renal failure were chosen as experimental group and 30healthy peo- ple as control group,and the serum levels of TIBC were analyzed.The ROC curve was used to evaluate the value of TIBC in the diagnosis of chronic renal failure.Kappa test was used to analyze the consistency of results between TIBC in the diagnosis of chronic renal failure and creatinine in that.Results The serum TIBC was (40.86-4-7.54)μmol/L in the chronic renal failure group,while that of the control group was (61.96-4-7.48)μmol/L,the difference was statisti- cally significant(t=10.87,P<:0.05).Under the ROC curve area of TIBC diagnosis of chronic renal failure was 0.979, and when the cut off value of TIBC was 49μmol/L,the diagnostic sensitivity was 86.7%and the specificity was 100%TIBC for chronic renal failure diagnosis and creatinine diagnosis had a good Consistency (Kappa=0.867,P< 0.05).Conclusion Serum total iron binding capacity (TIBC)has a high diagnostic efficacy in the diagnosis of chronic renal failure and can be used for auxiliary diagnosis of chronic renal failure.
