2型糖尿病患者高尿酸血症流行状况与相关因素研究

Study on prevalence and correlates of hyperuricemia in type 2 diabetic patients

目的探讨T2DM患者高尿酸血症(HUA)流行状况和相关因素。方法选取2008年1月至2013年3月于天津医科大学代谢病医院住院的T2DM患者2178例,根据SUA水平分为单纯糖尿病组(T2DM,n=1860)和HUA组(n=318),检测临床生化指标,分析HUA流行状况,Logistic回归分析T2DM患者HUA的影响因素。结果 2178例T2DM患者中检出HUA 318例,检出率为14.6%。Logistic回归分析显示,调整混杂因素后,女性、年龄、BMI、病程、DKD、HbA1c、TG、C-RP、BUN、Scr和微量白蛋白尿(UMalb)与HUA有关[OR(95%CI)分别为1.621(1.170～2.246)、1.192(1.044～1.361)、1.812(1.523～2.155)、1.257(1.070～1.477)、2.741(2.084～3.605)、0.445(0.318～0.621)、2.467(1.746～3.487)、1.670(1.167～2.390)、4.678(2.706～8.086)、2.593(1.251～5.374)和1.757(1.244～2.482)]。结论性别、年龄、病程、BMI、DKD、血糖、血脂等与T2DM患者HUA有相关性。

Objective To analyze the epidemic status and explore the correlates of hyperuricemia in patients with type 2diabetes mellitus (T2DM). Methods Data of general characteristic and serum biochemical indexes were collected in 2178 T2DM patients hospitalized in Metabolic Disease Hospital,Tianjin Medical University from January,2008to March,2013and the epidemic status of hyperuricemia was analyzed and of factors related to hyperuricemia were calculated using multivariate logistic regression compared with T2DM patients with normal uric acid.According to SUA level,they were divided into simple diabetes group (T2DM,n =1860)and HUA group (n =318). Results Among 2178T2DM patients,318patients had hyperuricemia,the prevalence of hyperuricemia was 14.6% .The results of multivariate logistic regression analysis showed that female sex,age,BMI,diabetic duration,diabetic kidney disease (DKD), HbA 1 c,TG,C-RP,BUN,Scr and urine microalbumin (UMalb)were related to the hyperuricemia in T2DM patients after controlled possible confounding factors.Their OR and 95% CI were 1.621 (1.170~2.246), 1.192 (1.044~1.361), 1.812 (1.523~2.155), 1.257 (1.070~1.477), 2.741 (2.084~3.605), 0.445 (0.318~0.621), 2.467 (1.746~3.487), 1.670 (1.167~2.390), 4.678 (2.706~ 8.086), 2.593 (1.251~5.374), 1.757 (1.244~2.482), respectively. Conclusion Gender,age,duration of T2DM patients,BMI,DKD,blood glucose,blood lipids and other factors were associated with hyperuricemia in T2DM patients.
