低切迹钢板与克氏针张力带治疗尺骨鹰嘴骨折的比较

Low-profile plate versus Kirschner wire tension band for ulna olecranon fracture

[目的]探讨低切迹钢板和克氏针张力带钢丝内固定治疗尺骨鹰嘴骨折的临床疗效。[方法] 2012年01月～2015年12月,52例尺骨鹰嘴骨折患者被随机分为两组,其中低切迹钢板内固定26例,克氏针张力带钢丝内固定26例。采用VAS疼痛评分评定疼痛程度,采用Mayo肘关节评分评估功能情况^([4]),评估记录肘关节活动度。[结果]两组患者均顺利手术,无血管、神经损伤等严重并发症。52例均获得随访,随访时间12～24个月,平均(18.71±4.59)个月。随访过程中,低切迹钢板组的并发症发生率略低于克氏针张力带组,但差异无统计学意义(P>0.05)。末次随访时,低切迹钢板组的VAS评分、Mayo评分及肘关节活动功能略优于克氏针张力带组,但差异无统计学意义(P>0.05)。[结论]对于尺骨鹰嘴骨折,低切迹钢板和克氏针张力带钢丝内固定治疗均能取得良好的临床疗效。

[Objective] To compare the clinical outcomes of low-profile plate versus Kirschner wire tension band for the treatment of ulna olecranon fracture. [Methods] From January 2012 to December 2015, 52 patients with ulna olecranon fracture were randomized into two groups. Of them, 26 patients had fracture fixed with low-profile plate(the plate group), while the remaining 26 patients was treated with Kirschner wire tension band(the tension band group). The visual analogue scale(VAS)for pain, Mayo score and elbow range of motion(ROM) were used for evaluation of the clinical consequences. [Results] All the patients had operation performed smoothly without serious complication, such as neurovascular injuries. All of them were followed up for 12~24 months with an average of(18.71±4.59) months. During the follow up, the plate group proved slightly less complication than the tension band group, although no a statistically significant difference was noticed between the two groups(P>0.05). At the latest follow up, the plate group was superior to the tension band group to some extent regarding VAS, Mayor Elbow score and ROM, despite no a statistically significant difference between them(P>0.05). [Conclusion] Both the low-profile plate and the Kirschner wire tension band are effective treatment for ulna olecranon fracture with satisfactory clinical results.
