综合康复治疗对成人锁骨骨折患者内固定术后肩关节功能的影响

The influence of comprehensive rehabilitation therapy on shoulder joint function of adult patients with clavicle fracture after internal fixation

目的:观察综合康复治疗对成人锁骨骨折患者内固定术后肩关节功能的影响。方法:选取2010年9月~2015年9月本院实施成人锁骨骨折内固定术患者60例,随机分为治疗组及对照组各30例。两组患者均根据锁骨骨折不同分型实施内固定手术治疗,术后治疗组在临床常规治疗的基础上实施综合康复治疗,对照组患者术后给予临床常规治疗,患者自己进行辅助训练。对两组患者进行12个月的随访,观察其术后骨折愈合情况及肩关节功能恢复的情况。骨折愈合情况的观察采用肩关节X线片,肩关节功能采用Neer评分方法,并对两组患者术后第12个月的肩关节功能Neer评分进行比较。结果:治疗组术后12个月肩关节功能Neer评分显著高于对照组(P<0.05)。治疗组优良率显著高于对照组(P<0.05)。结论:综合康复治疗可促进成人锁骨骨折内固定术后肩关节功能的恢复,可作为成人锁骨骨折内固定术后极其重要的治疗手段。

Objective: To observe the influence of comprehensive rehabilitation therapy on shoulder joint function of adult patients with clavicle fracture after internal fixation. Method: From September 2010 to September 2015, 60 patients with adult clavicular fracture after internal fixation were randomly divided into treatment group(n=30) and control group(n=30). Internal fixation was operated on both two groups according to different type of clavicle fracture. On the basis of clinical routine treatment, patients of treatment group were given comprehensive rehabilitation therapy, and Patients of the control group did auxiliary training by themselves. Both two groups of patients were followed up for 12 months and postoperative healing and recovery of shoulder function were observed. Fracture healing was observed with shoulder joint x-ray. Shoulder joint function was observed by Neer score. After 12 months, Neer score were compared between two groups. Result: The Neer score of shoulder joint function in the treatment group was significantly higher than that in the control group 12 months after operation(P<0.05). The excellent and good rate in the treatment group was significantly higher than that in the control group(P<0.05).Conclusion: Comprehensive rehabilitation therapy can promote the shoulder joint function of adult patients with clavicle fracture after internal fixation, which could be an extremely important treatment method for adult patients with clavicle fracture after internal fixation.
