尹建平治疗湿热病用药规律聚类分析

Cluster Analysis on YIN Jianping’s Medication Rule in Treatment of Dampness-heat Disease

目的 :探讨尹建平治疗湿热病的用药规律。方法 :收集尹建平门诊治疗湿热病有效或显效病案的中药处方,利用Excel软件对处方中中药进行频数统计,取累积频率在90%以内的中药作为高频中药,采用SPSS 20.0软件对高频中药进行聚类分析。结果:共收集了444例病案,总计中药处方500首,共使用中药136味,其中高频中药40味,出现频率前5位的为苍术、甘草、黄连、厚朴、干姜,对高频中药聚类分析得到6个聚类方,C1:苍术、甘草、黄连、厚朴;C2:干姜、藿香;C3:半夏、陈皮、薏苡仁;C4:茯苓、西洋参、黄芩;C5:黄柏、茵陈、桑白皮、白术;C6:神曲、救必应、砂仁。结论:聚类分析结果客观反映了尹建平治疗湿热病分消湿热、宣肺开闭、降肺气利湿、行气通阳、辛寒降湿热、顾护中阳以及湿热病后期调阳补阴的用药规律。

Objective:To explore YIN Jianping’s medication rule in the treatment of damp-heat disease.Methods:Prescriptions for the treatment of patients with damp-heat disease who were improved or cured in YIN Jianping’s effective prescriptions of cases were collected and the frequency of used medicinal herbs were calculated using Excel software. Taking medicinal herbs with cumulative frequency within 90% as the high-frequency medicinal herbs,the high-frequency medicinal herbs were carried out cluster analysis by SPSS20.0 software. Results:A total of 444 cases were collected,with total 500 prescriptions and total 136 medicinal herbs,including 40 high frequency medicinal herbs. The top 5 high-frequency medicinal herbs were Cangzhu(Atractylodis Rhizoma),Gancao(Glycyrrhizae Radix Et Rhizoma),Huanglian(Coptidis Rhizoma),Houpu(Magnolia Officinalis Cortex),and Ganjiang(Zingiberis Rhizoma). Total 6 cluster prescriptions of high frequency medicinal herbs were found,including C1:Cangzhu(Atractylodis Rhizoma),Gancao(Glycyrrhizae Radix Et Rhizoma),Huanglian(Coptidis Rhizoma),Houpu(Magnolia Officinalis Cortex);C2:Ganjiang(Zingiberis Rhizoma) and Huoxiang(Pogostemonis Herba);C3:Banxia(Pinellia Rhizoma),Chenpi(Citri Reticulatae Pericarpium),Yiyiren(Coicis Semen);C4:Fuling(Poria),Xiyangshen(Panax quinquefolium Radix),Huangqin(Scutellaria Radix);C5:Huangbo(Phellodendri Chinensis Cortex),Yinchen(Artemisiae Scopariae Herba),Sangbaipi(Mori Cortex),Baizhu(Atractylodis Macrocephalae Rhizoma);C6:Shenqu(Divine Comedy),Jiubiying(Ilicis Rotundae Cortex),Sharen(Amomi Fructus). Conclusions:The results of cluster analysis objectively reflected YIN Jianping’s medication rules in treating damp-heat,ventilating the lung qi,reducing lung qi,eliminating the dampness,moving qi and activating yang,using herbs pungent in taste and cold in nature to reduce dampness and heat,protecting middle yang and regulating yin and yang in the advanced period of damp-heat disease.
