胰腺实性假乳头状瘤的CT与MRI特点

CT and MRI Features of Solid Pseudopapillary Tumor of Pancreas

目的:通过总结胰腺实性假乳头状瘤(SPTP)的CT及MRI特点,减少SPTP的误诊率,提高患者生存质量。方法:回顾性分析经病理确诊的9例SPTP的CT及MRI图像。结果:9例单发。胰头3例,胰腺尾部4例,胰腺颈部1例,胰腺体部1例。均为类圆形,最大长径50mm。实性成分平扫CT值约36.6HU,增强扫描呈渐进性强化,动脉期CT值约63.0HU,门静脉期CT值约77.8HU。T1WI呈低信号为主,T2WI呈明显高信号,DWI呈高信号,ADC图呈低信号,增强扫描呈渐进性强化。结论:SPTP具有一定的临床及影像学特点,有助于与其它胰腺肿瘤进行鉴别诊断。

Purpose: To reduce the misdiagnosis rate of solid pseudopapillary tumor of pancreas (SPTP) through summarizing the CT and MRI features of SPTP. Methods: CT and MRI images of 9 cases of SPTP confirmed by pathology results were analyzed retrospectively. Results: Nine cases revealed solitary nodule, in which 3 cases located in pancreatic head, 4 cases in pancreatic tail, 1 case in pancreatic neck and 1 case in pancreatic body. The morphological characteristics of them were round shape and the maximum length was 50mm. CT value of solid component was about 36.6 HU. On contrast-enhanced CT scan, the tumor showed progressive enhancement. The CT value in arterial phase was about 63.0 HU, and about 77.8 HU in portal vein phase. MRI showed that the tumor was with low intensity signal on T1WI, while with high intensity signal on T2WI. On DWI, it showed with high intensity signal and on ADC map with low intensity signal. On contrast-enhanced MRI it also showed progressive enhancement. Conclusion: SPTP has certain clinical and morphological features which are helpful for differentiating SPTP from other pancreatic tumors.
