天轨步行结合头皮针治疗脑梗死患者下肢运动功能障碍

Treatment of Lower Limb Movement Dysfunction in Patients with Cerebral Infarction by Overhead Orbital Walking Combined with Scalp Acupuncture

目的:观察天轨步行结合头皮针治疗脑梗死后下肢运动功能障碍的总体疗效,为脑梗死患者下肢运动功能障碍的治疗寻求一种更为有效的方法。方法:将符合纳入标准的120例脑梗死后下肢运动功能障碍患者随机分为天轨步行组、头皮针治疗组和联合治疗组(n=40)。天轨步行组采用常规天轨步行治疗,头皮针治疗组采用常规头皮针治疗,天轨步行结合头皮针联合治疗组采用头皮针治疗,带针状态下予以天轨步行治疗,10 d为1个疗程,各组治疗3个疗程。治疗前后3组患者采用Fugl Meyer运动功能量表评定其下肢运动功能,Berg平衡量表评定其平衡功能,Barthel指数评定其日常生活能力。结果:治疗后,3组患者各疗程的FMA、BBS及Barthel指数评分均较治疗前有明显的提高,差异具有显著性(P<0.05);且同一疗程联合组的各项评分均显著高于天轨步行组和头皮针组(P<0.05)。结论:天轨步行与头皮针结合治疗脑梗死患者下肢运动功能障碍疗效确切,值得推广。

Objective To observe the effect of overhead orbital walking combined with scalp acupuncture on lower limb movement impairment after cerebral infarction,and to seek a more effective method for the treatment of lower limb movement impairment after cerebral infarction. Methods 120 patients with lower limb movement impairment after cerebral infarction who met the inclusion criteria were randomly divided into the overhead orbital walking group,the scalp acupuncture treatment group and overhead orbital walking combined with scalp acupuncture treatment group( n = 40). The overhead orbital walking group was treated with conventional astronomical walking. The scalp acupuncture treatment group was treated with conventional scalp acupuncture,and the combined group was treated by scalp acupuncture at first,and overhead orbital walking treatment was performed with the needle. The 10 d was a course of treatment,and each group was treated for 3 courses. Before and after treatment,the Fugl Meyer motor function scale was used to evaluate the lower limb motor function,the Berg balance scale to assess its balance function,and the Barthel index to assess its daily living ability. Results After treatment,the FMA,BBS and Barthel index scores of the 3 groups were significantly higher than those before treatment( P<0.05); and the scores in the combined group were significantly higher than those in the overhead orbital walking group and scalp acupuncture group( P<0.05). Conclusion Overhead orbital walking combined with scalp acupuncture in the treatment of lower limb motor dysfunction in patients with cerebral infarction is effective and worthy of promotion.
