中医药治疗恶性腹水证候及用药规律分析

Rule Analysis of TCM Syndromes and Medications in Treatment of Malignant Ascites

目的:通过文献检索探讨中医药治疗恶性腹水证候及用药规律分析。方法:检索中国期刊全文数据库2000年1月至2015年12月相关文献,纳入文献63篇,分析文献涉及的主要证型、治法及用药规律,并对常用的单味药进行总结。结果:恶性腹水的辨证论治体系中以阳虚多见,脏腑关系最为密切的为肝、脾、肾,气血与腹水的发生关系亦很密切,中医辨证分型以脾肾阳虚证最为多见,而所应用的中药处方中经典方相对较少,自拟方较多;所提及的中医治法,利水渗湿法、健脾益气法、温阳化气法、理气消胀法、活血利水法所占频次为前5位,出现频数> 6次的单味药前5位为利水渗湿药、补气药、理气药、温里药、活血化瘀药。结论:通过文献整理为临床辨证论治恶性腹水提供一定的理论依据。

Objective: To explore the rule of treatment of malignant ascites in syndrome and drug use with the literature searching method. Methods: The relevant literatures of Chinese Journal Full-text Database from January 2000 to December 2015 were searched,and 63 articles were included in the literature. The main types of syndromes,treatment methods and medication rules involved in the literature were analyzed,and the commonly used single-flavor drugs were summarized. Results: In the syndrome differentiation and treatment system of malignant ascites,Yang deficiency Syndrome was more common. The most closely related Zang was Liver,Spleen and Kidney. The relationship between Qi and Blood and the happening of ascites was very close. The most common syndrome in the syndrome differentiation of TCM was Spleen and Kidney Yang deficiency. However,there are relatively few classic prescriptions used in treatment,and there are more self-prepared prescriptions. Among the mentioned Chinese medicine treatment methods,Lishui Shenshi method,Jianpi Yiqi method,Wenyang Huaqi method,Yiqi Xiaozhang method and Huoxue Lishui method were top five methods. The single-flavor medicine with frequency > 6 times and the top 5 single-flavored medicine are Lishui Shenshi medicine,Buqi medicine,Liqi medicine,Wenli medicine and Huoxue Huayu medicine. Conclusion: The literature search method provides a theoretical basis for clinical syndrome differentiation and treatment of malignant ascites.
