摘要
目的研究经股动脉逆行途径射频消融His束建立完全性房室传导阻滞(AVB)动物模型的可行性。方法 15~20kg健康雄性杂种犬8只,麻醉开胸后于左室缝制临时起搏电极及记录电极。在X线引导下,将消融电极经股动脉逆行至主动脉无冠窦,标测定位并射频消融His束。分别记录每次实验的操作和消融时间,比较消融前后的心率及心律变化,观察术后并发症情况。结果 8只犬成功建立完全性AVB动物模型,操作时间17.5(1,25.25)min,消融时间84(10,90)s。所有犬在消融前均为窦性心律,心率(168±13)次/分,消融时出现交界性心律,消融完成后体表及腔内心电图均显示完全性AVB,心率(57±11)次/分。结论通过股动脉逆行途径射频消融His束建立完全性房室传导阻滞动物模型的方法简便可行、成功率高。
Objective To explore the feasibility of a new method to establish complete atrioventricular block animal model induced by radiofrequency catheter ablation through femoral artery. Methods Eight healthy male mongrel dogs weighing 15-20 kg were anesthetized with 3% Na-pentobarbital and sutured with temporary pacing electrodes and recording electrodes on the left ventricle.Under the guidance of X-ray,the ablation catheter was retrogradely delivered to the noncoronary cusp via femoral artery.Once the potential of His bundle was recorded,radiofrequency energy has been delivered to complete the ablation of the His bundle.The operation and ablation time of each experiment were recorded separately.Observing and comparing the changes of heart rate and rhythm,postoperative complications before and after ablation. Results All dogs were successfully established the complete atrioventricular block animal model.The procedure time was 17.5(1,25.25) minute and the ablation time was 84(10,90) second.All dogs were sinus rhythm before ablation and the mean heart rate was (168±13) bpm.After the ablation was completed,the surface and intracavitary ECG showed atrioventricular block and the mean heart rate was (57±11)bpm.Junctional rhythm appeared in all dogs during ablation. Conclusion Radiofrequency ablation of His bundles via femoral artery retrograde approach is a new feasible method with high-success rate for establishing a complete atrioventricular block animal model.
