先天性扣拇畸形的分类及其治疗

A clinical analysis of classification and treatment of congenital clasped thumb

目的探讨先天性扣拇畸形的分类,以及不同分类的治疗方法及治疗效果。方法选用2010年1月至2018年1月我科诊治的113例(207个拇指)先天性扣拇畸形患者,将患者按不同的病理解剖特点分为6类,并按不同分类予以相应的治疗。结果 97例(150个拇指)获平均14.6个月的随访,优良率74.0%(111/150),且未发生植皮不成活、挛缩复发等不良并发症。结论本分类法对于先天性扣拇畸形的治疗有指导意义,以此分类法进行的个体化治疗,临床疗效满意,有推广意义。

Objective To propound a new classification of congenital clasped thumb, to summarize a systematic treatment strategy, and to evaluate the treatment effects. Methods A total of 113 patients with 207 affected thumbs were treated by hand surgery department in Jishuitan Hospital from January 2010 to January 2018. All patients were divided into 6 categories by different anatomic changes, and were treated by relevant treatment schemes. Results After treatment, 97 patients with 150 thumbs were followed up for a mean of 14.6 months, and the excellent and good rate was 74.0% (111/150). No skin graft necrosis or contracture occurred. Conclusions The classification method described in this article has a systematic guidance for the treatment of congenital clasped thumb. Thus, patients with congenital clasped thumb should be divided by this classification method, and treated relevantly. If so, satisfactory treatment results will be achieved.
