腹膜透析、血液透析两种透析方式对慢性肾衰竭终末期患者微炎症及营养状态的影响观察 被引量:14

摘要 目的探究腹膜透析、血液透析两种透析方式对慢性肾衰竭终末期患者微炎症及营养状态的影响。方法 70例慢性肾衰竭终末期患者,根据随机数字表法分为A组和B组,每组35例。A组患者采用腹膜透析治疗, B组患者采用血液透析治疗。对比两组患者治疗前后微炎症状态、贫血状态纠正情况、营养状态。结果治疗后,两组患者白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)、超敏C反应蛋白(hs-CRP)水平均较本组治疗前改善,且A组IL-6(14.92±2.48)ng/L、TNF-α(14.16±4.31)ng/L、hs-CRP(5.52±1.23)mg/L均优于B组的(19.38±3.12)ng/L、(18.27±3.18)ng/L、(7.18±1.76)mg/L,差异具有统计学意义(P<0.05)。两组患者血红蛋白(Hb)、红细胞计数(RBC)、红细胞压积(Hct)水平均较本组治疗前改善,且A组改善程度优于B组,差异具有统计学意义(P<0.05)。治疗后,两组患者血肌酐、血白蛋白及尿量水平均较本组治疗前改善,且A组改善程度优于B组,差异具有统计学意义(P<0.05)。结论临床治疗慢性肾衰竭终末期患者采用腹膜透析能够使其微炎症及营养状态得到更明显地改善,值得推广。 Objective To discuss the effect of peritoneal dialysis and hemodialysis on microinflammation and nutritional status in patients with end-stage chronic renal failure. Methods A total of 70 patients with end-stage chronic renal failure were divided by random number table method into group A and group B, with 35 cases in each group. Group A was treated with peritoneal dialysis, and group B was treated with hemodialysis. Comparison were made on microinflammation, anemia status correction, and nutritional status before and after treatment between the two groups. Results After treatment, both groups had improved interleukin-6(IL-6), tumor necrosis factor-α(TNF-α) and hypersensitive C-reactive protein(hs-CRP) level than those before treatment, and group A had better IL-6 as(14.92±2.48) ng/L, TNF-α as(14.16±4.31) ng/L and hs-CRP as(5.52±1.23) mg/L than(19.38±3.12) ng/L,(18.27±3.18) ng/L and(7.18±1.76) mg/L in group B, and the differences were statistically significant(P<0.05). Both groups had improved hemoglobin(Hb), red blood cell count(RBC), hematocrit(Hct) level than those before treatment, and group A had better improvement degree than group B. the differences were statistically significant(P<0.05). After treatment, both groups had improved serum creatinine, serum albumin and urine volume than those before treatment, and group A had better improvement degree than group B. The differences were statistically significant(P<0.05). Conclusion Peritoneal dialysis can significantly improve the microinflammation and nutritional status of patients with end-stage chronic renal failure, and it is worthy of promotion.
