药物治疗管理服务模式在不同来源糖尿病患者中的应用和探讨

通过药师为门诊、住院、社区等不同来源的糖尿病患者提供药物治疗管理(medication therapy management, MTM)服务和随访的典型案例,探讨不同患者对MTM服务的需求方向,以及药师在为他们提供MTM服务时应关注的切入点和技巧。通过实践发现,门诊咨询、社区糖尿病患者对MTM服务的需求面更广,存在药物相关问题较多,如用药疗程、重复用药、药物剂量等。药师在面对他们时应注意信息搜集的全面性,注意与医护人员的配合。对住院患者应侧重于生活方式、监测方面的建议;对门诊及社区患者,应侧重于发现药物相关问题,增加用药依从性。MTM服务可以在不同场所开展,而掌握不同来源患者的特点,有助于提高MTM服务的效率。

Typical medication therapy management(MTM) services cases were analyzed including diabetic patients from outpatient, inpatient and community. Requirement and skills for MTM services in those typical cases were identified. Through practice, it is found that diabetic patients from outpatient and community have a wider demand for MTM services, and there are more drug-related problems, such as course of treatment, repeated use of drugs, drug dosage. Pharmacists should pay attention to the comprehensiveness of information collection and cooperation with medical staff. Suggestions on lifestyle and monitoring should be given to inpatients, and more attention should be paid to finding drug-related problems and increasing drug compliance for outpatients and patients from community. MTM services can be carried out in different locations. Grasping the characteristics of patients from different sources will help improving the efficiency of MTM services.
