牙周炎患者基础治疗前后唾液和龈沟液中IL-6、TNF-α、MMP-8水平的变化

Changes of IL-6,TNF-α and MMP-8 levels in saliva and gingival crevicular fluid before and after basic treatment in patients with periodontitis

目的观察牙周炎患者基础治疗前后唾液及龈沟液中IL-6、TNF-α、MMP-8水平的变化及其与牙周临床指标的相关性,探讨IL-6、TNF-α、MMP-8在牙周炎发生发展中的作用。方法采集40例慢性牙周炎患者作为治疗组和25名健康者作为对照组,收集两组基础治疗前后静息全唾液和龈沟液,用酶联免疫吸附分析法(ELISA)检测基础治疗前后唾液、龈沟液中IL-6、TNF-α、MMP-8的含量,同时记录治疗前、后患牙的探诊深度(PD)、附着丧失(AL)、龈沟出血指数(SBI)和菌斑指数(PLI),并分析其相关性。结果治疗组治疗前牙周临床指标均明显高于对照组(P <0. 05),经基础治疗后,各指标较治疗前显著下降(P <0. 05)。治疗组基础治疗前后唾液和龈沟液中IL-6、TNF-α、MMP-8均高于对照组,治疗后较治疗前三者表达均降低。同时治疗前龈沟液中IL-6、TNF-α、MMP-8与牙周临床指标有显著相关性(P <0. 05),治疗后IL-6与牙周组织SBI、PLI无相关性(P> 0. 05)。结论慢性牙周炎疾病发生发展与唾液、龈沟液中IL-6、TNF-α、MMP-9浓度有着密切关系,可以作为提示牙周炎严重程度的信号因子。

Objective To investigate the changes of IL-6,TNF-α and MMP-8 levels in saliva and gingival crevicular fluid before and after basic treatment in patients with periodontitis and its correlation with clinical periodontal index. Methods Forty patients with chronic periodontitis were included in this study and 25 healthy subjects served as control. Saliva and gingival crevicular fluid were collected before and after the treatment and IL-6,TNF-α and MMP-8levles were examined by ELISA. The periodontal indexes PD,AL,SBI and PLI were recorded and their correlation was analyzed. Results The periodontal indexes were significantly higher in the treatment group than those in the control( P <0. 05). After the basic treatment,the indexes were significantly lower than those before treatment( P < 0. 05). IL-6,TNF-α and MMP-8 in the treatment group were higher than those in the control before and after treatment. There was a significant correlation between IL-6,TNF-α,MMP-8 and periodontal clinical indexes( P < 0. 05). Conclusion The development of chronic periodontitis is closely related to IL-6,TNF-α and MMP-9 leves in saliva and gingival crevicular fluid,which can be used as the markers for the severity of periodontitis.
