NSTE-ACS患者临界病变的血管内超声显像特征

Characteristics of Intravascular Ultrasound Imaging in Coronary Critical Lesions of Patients with NSTE-ACS

目的探讨NSTE-ACS患者冠脉造影提示为临界病变的血管内超声(IVUS)显像特征。方法入选冠脉造影存在临界病变的NSTE-ACS患者72例,共88处临界病变进行IVUS检查,观察斑块的定性及定量特点。结果 NSTE-ACS患者临界病变斑块IVUS提示大部分为偏心斑块及混合斑块,冠脉造影管腔狭窄率与IVUS斑块负荷之间虽然存在直线相关关系(r=0. 914,P <0. 05),但冠脉造影低估了冠脉狭窄程度(61. 7±8. 3%VS 69. 2±9. 3%,P <0. 05)。结论冠脉造影提示为临界病变的斑块,仍存在一定比例患者应行介入治疗,IVUS可成为准确评估临界病变狭窄程度及指导治疗策略的有效方法。

Objective To Study the intravascular ultrasound (IVUS)imaging characteristics of critical lesions indicated by coronary angiography (CAG)in NSTE -ACS patients.Methods Using IVUS to observe the qualitative and quantitative coronary plaque characteristics of 72NSTE -ACS patients indicated by CAG.Results The IVUS showed that most of the critical lesions plaque in the NSTE -ACS patients were eccentricity plaque and mixed plaque.Although there was a linear correlation between coronary stenosis rate and plaque load of IVUS in CAG (r = 0.914,P <0.05),CAG underestimated the degree of coronary stenosis (61.7±8.3% VS69.2±9.3%,P <0.05). Conclusions There is still a certain proportion of patients who had coronary critical lesions indicated by CAG should be treated with interventional therapy.IVUS can be an effective method to evaluate the degree of critical stenosis and guide the treatment strategy.
