摘要
目的分析2013-2017年中国人感染H7N9禽流感病毒者死亡的相关风险因素。方法通过"万方数据库"、"中国知网(China National Knowledge Infrastructure,CNKI)"、"维普资讯网"进行检索,收集2013-2017年公开发表的关于H7N9禽流感病例流行病学特征的文献,应用Review Manager 5.0软件对死亡病例的流行病学特征进行Meta分析。结果纳入符合标准的文献20篇,Meta分析结果显示,与女性相比,男性感染H7N9死亡的可能性更高,OR(95%CI)=1.57(1.09~26.00);与其他年份相比,2015年人感染H7N9死亡的可能性更高,OR(95%CI)=2.02(1.23~3.31);与≥60岁组相比,0~14岁和15~59岁组死亡的可能性更低,OR(95%CI)值分别为0.14(0.04~0.52)和0.50(0.29~0.84);与无明确禽类接触史者相比,有接触史者死亡的可能性更高,OR(95%CI)=1.88(1.02~3.45);与无慢性病史者相比,有慢性病史者死亡的可能性更高,OR(95%CI)=8.63(1.44~51.7)。结论年龄、活禽市场暴露、有慢性基础疾病史是H7N9感染死亡的相关风险因素。
ObjectiveTo analyze the risk factors for the death cases of human infected with avian influenza A (H7N9) virus (H7N9 cases) during 2013 to 2017 in China.MethodsArticles with topics on epidemiological features of H7N9 cases published during 2013 to 2017 were screened out from databases, including CNKI, Wanfang Data, VIP database and PubMed. The epidemiologic features of death cases were investigatedby Review Manager 5.0 software.ResultsTwenty articles were enrolled in analysis according to inclusion criteria. Meta-analysis results indicated higher probability of death among male H7N9 cases than females with the OR (95%CI) as 1.57(1.09-26.00). The probability of death among H7N9 case in 2015 was higher than that in other years with the OR (95%CI) as 2.02 (1.23-3.31). Comparing to cases in ≥60 years old group, probability of death among cases in 15-59 years old group and 0-14 years old group were lower with the OR (95%CI) as 0.14 (0.04-0.52) and 0.50 (0.29-0.84), respectively. The probability of death among cases with poultry exposure was higher than those without contact history with the OR (95%CI) as 1.88 (1.02-3.45). For cases with chronic illnesses, probability of death was higher than those without chronic diseases with the OR (95%CI) as 8.63 (1.23-3.31).ConclusionsAge, exposure in live poultry market and chronic illnesses were the risk factors related to death cases of avian influenza A(H7N9) virus in human.
