摘要
【目的】研究芪棱汤对2型糖尿病并发脑梗死患者氧化应激和炎症反应的影响。【方法】收集2型糖尿病并发脑梗死患者160例,采用随机数字表将患者随机分为对照组和研究组,每组各80例。对照组给予西医常规方案治疗,研究组在对照组西医常规方案治疗的基础上加用芪棱汤治疗,疗程30 d。评估2组患者治疗后神经功能缺失的改善疗效,检测2组患者治疗前后血清中氧化应激相关因子[超氧化物歧化酶(SOD)、谷胱甘肽过氧化物酶(GSH-Px)和丙二醛(MDA)]和炎性因子[白细胞介素6(IL-6)、肿瘤坏死因子α(TNF-α)、白细胞介素1β(IL-1β)]水平。【结果】(1)神经功能缺损的改善疗效:治疗30 d后,研究组的总有效率为98.75%,对照组为93.75%,研究组的疗效优于对照组(P <0.05)。(2)治疗后,2组患者血清SOD、GSH-Px水平均升高(P <0.05),MDA水平均降低(P <0.05),且研究组在升高血清SOD、GSH-Px水平和降低血清MDA水平方面均明显优于对照组(P <0.05)。(3)治疗后,2组患者血清IL-6、TNF-α和IL-1β水平均较治疗前降低(P <0.05),且研究组对血清IL-6、TNF-α和IL-1β水平的降低作用均明显优于对照组(P <0.05)。【结论】芪棱汤能够有效提高2型糖尿病并发脑梗死患者机体的抗氧化能力,降低机体内的炎症反应水平,改善患者的神经功能。
Objective To explore the influence of Qileng Decoction on the oxidative stress and inflammatory reaction in patients with type 2 diabetes mellitus complicated with cerebral infarction. Methods One hundred and sixty patients with type 2 diabetes mellitus complicated with cerebral infarction were randomly divided into control group and study group according to the random number, 80 cases in each group. The basic regimen of type 2 diabetes complicated with cerebral infarction was adopted for the control group,and the study group received the basic regimen plus Qileng Decoction, the treatment lasting for 30 d. The clinical therapeutic efficacy on neurological deficit in the two groups were evaluated after treatment, and the levels of serum oxidative stress factors such as superoxidase dismutase(SOD),glutathione peroxidase(GSH-Px)and malondialdehyde(MDA)as well as inflammatory factors of interleukin 6(IL-6),tumor necrosis factor alpha(TNF-α)and interleukin 1 beta (IL-1β)in the two groups before and after treatment were detected. Results(1)After treatment,the total effective rate for improving neurological deficit in the study group was 98.75% and that in the control group was 93.75%, the difference being significant(P < 0.05).(2)The levels of serum SOD and GSH-Px were increased significantly and MDA level was decreased in the two groups after treatment(P < 0.05 compared with those before treatment), and the effect on increasing SOD and GSH-Px levels and on decreasing MDA level in the study group was superior to that in the control group(P < 0.05).(3)After treatment,the levels of serum IL-6,TNF-α,and IL-1β in the two groups were decreased significantly(P < 0.05 compared with those before treatment),and the decrease in the study group was superior to that in the control group(P < 0.05). Conclusion Qileng Decoction can effectively improve the anti- oxidation ability of the patients with type 2 diabetes mellitus complicated with cerebral infarction,reduce the inflammatory reaction,and improve the nerve function.
