针药结合中医特色护理对气血亏虚型产妇产后缺乳的预防效果

Preventive Effect of Specific Traditional Chinese Medicine Nursing Measures of Acupuncture Plus Herbs on Postpartum Hypogalactia in Puerperas with Qi-blood Deficiency Type

【目的】探讨针药结合为主的中医特色护理方法对气血亏虚型产妇产后缺乳的预防效果和护理措施。【方法】将138例产妇分为观察组和对照组,每组69例,对照组产妇采用常规西医护理,观察组产妇在对照组的基础上配合针药结合(针刺+口服养血生乳汤)为特色的中医护理。比较2组产妇泌乳开始时间、不同泌乳时间段内的产妇人数、血清泌乳素水平以及乳房充盈情况的差异,比较2组42 d时母乳喂养的成功率。【结果】观察组产妇泌乳开始时间显著低于对照组(P<0.05),24 h内泌乳的产妇数量显著高于对照组(P<0.05);观察组产妇产后48 h和产后72 h血清泌乳素水平显著高于对照组(P<0.05);观察组产妇乳房充盈程度为Ⅰ级者比例高于对照组(P<0.05);观察组母乳喂养成功率为91.30%,高于对照组的85.71%(P<0.05)。【结论】以针药结合为特色的中医护理,在气血亏虚型产妇的产后护理中,可以有效地促进乳汁分泌,改善乳房充盈程度,提高母乳喂养成功率。

Objective To explore the preventive effect of specific traditional Chinese medicine nursing measures ofacupuncture plus herbs on Qi-blood deficiency type of postpartum hypogalactia in the puerperas. Methods A totalof 138 cases of qualified parturient women were divided into observation group and control group,69 cases in eachgroup. The control group received routine western medicine nursing,while the observation group received specifictraditional Chinese medicine nursing measures of acupuncture plus oral use of Yangxue Shengru Decoctioncombined with routine western medicine nursing. Time for initiating lactation,the number of puerperas at variousinitiating lactation time,serum prolactin level and breast filling in the two groups were compared,and the breastfeeding rate in the two groups was compared on postpartum day 42. Results The results showed that the time forinitiating lactation was significantly earlier(P < 0.05),and the number of lactating women within 24 h in theobservation group was significantly more than that in the control group(P < 0.05). The serum prolactin level atpostpartum hour 48 and 72 in the observation group was significantly higher than that in the control group(P <0.05). The proportion of grade I breast filling in the observation group was higher than that in the control group(P <0.05),and the breastfeeding rate in the observation group was 91.30%,significantly higher than 85.71% in thecontrol group(P < 0.05). Conclusion The specific traditional Chinese medicine nursing measures of acupunctureplus herbs are effective on promoting postpartum lactation of the puerperas with Qi-blood deficiency type,improving breast filling, and enhancing the breastfeeding rate, which is worthy of further research andpopularization.
