刘凤斌辨治肝硬化用药规律探讨

Discussion on Medication Rules for Professor LIU Feng-Bin in Treating Liver Cirrhosis

采用频次分析和关联规则分析的方法,对刘凤斌教授2013年至2016年期间门诊治疗肝硬化的处方进行分析,探讨其组方用药规律。结果显示:收集到的合格的90首门诊处方中,共用药84味。频次居前的核心药物为白术、鸡内金、赤芍、当归、甘草、丹参、五指毛桃、茯苓、黄芪、太子参,药物组合以四君子汤为基础方加味疏肝化瘀药,共得到11首新方。分析结果提示刘凤斌教授治疗肝硬化多采用疏肝调肝、养肝柔肝之品以恢复"肝主疏泄"、"肝藏血"之生理特性;再根据治未病思想,以四君子汤加黄芪、五指毛桃健脾益气,防治脏腑之传变;另佐以破气、利水、通络、清热利湿等中药,随症加减。

Frequency analysis and association rul,e analysis were used for the investigation of the recipes of Professor LIU Feng-Bin prescribed for the liver cirrhosis outpatients in the years of 2013-2016,and then the medication rules were mined.The results showed that 90 recipes involving 84 herbs were obtained,the frequentlyused leading herbs were Rhizoma Atractylodis Macrocephalae,Endothelium Corneum Gigeriae Galli,Radix Paeoniae Rubra,Radix Angelicae Sinensis,Radix Glycyrrhizae Preparata,Radix Salviae Miltiorrhizae,Radix Fici Hirtae,Poria,Radix Astragali and Radix Pseudostellariae,herbal combination was characterized as Sijunzi Decoction combining with liver-soothing and stasis-removing herbs,and 11 new candidate recipes were obtained. The results indicated that the therapeutic principles of Professor LIU Feng-Bin in treating liver cirrhosis are to recover the function of liver for ensuring the free movement of Qi and for storing blood with herbs for soothing, regulating and nourishing liver,prevent the transmission of the affection of zang-fu organs by Sijunzi Decoction with Radix Astragali and Radix Fici Hirtae added following the theory of preventive treatment,and modify the basic recipe according to the alternation of symptoms with herbs for breaking stagnant Qi,inducing diuresis, unblocking collaterals,clearing heat and promoting dieresis.
