加减抵挡汤对痰瘀型糖尿病患者胰岛素抵抗的影响分析

Analysis of the Influence of Modified Didang Decoction on Insulin Resistance in Patients with Phlegm and Blood Stasis Type Diabetes Mellitus

目的研究加减抵挡汤对痰瘀型糖尿病患者胰岛素抵抗的影响。方法选取2015年2月—2017年3月痰瘀型糖尿病患者92例分两组,西医组采用常规饮食控制、运动锻炼,并给予胰岛素皮下注射治疗;中药组在西医组基础上给予加减抵挡汤治疗。就两组患者治疗前后胰岛素抵抗指数、胰岛素敏感指数、空腹血糖、糖化血红蛋白、胰岛β细胞功能和痰瘀型糖尿病患者胰岛素抵抗治疗总有效率、并发症发生率进行比较。结果中药组痰瘀型糖尿病患者胰岛素抵抗治疗总有效率明显高于西医组,P<0.05。两组患者均无严重并发症发生,P>0.05。治疗前两组患者胰岛素抵抗指数、胰岛素敏感指数、空腹血糖、糖化血红蛋白、胰岛β细胞功能相似,P>0.05。中药组患者治疗后胰岛素抵抗指数、胰岛素敏感指数、空腹血糖、糖化血红蛋白、胰岛β细胞功能均明显优于西医组,P<0.05。结论加减抵挡汤对痰瘀型糖尿病患者胰岛素抵抗的影响大,可有效改善患者胰岛β细胞功能和胰岛素抵抗,降低血糖水平,无严重并发症,安全有效,值得推广。

Objective To study the influence of modified Didang decoction on insulin resistance in patients with phlegm and blood stasis type diabetes mellitus.Methods 92 cases of patients with phlegm and blood stasis type diabetes mellitus from February 2015 to March 2017 92 were divided into two groups.The Western medicine group was treated with routine diet and exercise,and was given subcutaneous injection of insulin therapy.The traditional Chinese medicine group was given modified Didang decoction on the basis of Western medicine group. Before and after treatment,insulin resistance index,insulin sensitivity index,fasting blood glucose,glycosylated hemoglobin,β cell function and insulin resistance,therapeutic effect and complication rate were ebmpared.Results The total effective rate of the traditional Chinese medicine group was significantly higher than that of the Western medicine group,P <0.05.There were no serious complications in the two groups,P >0.05.The insulin resistance index,insulin sensitivity index,fasting blood glucose,glycosylated hemoglobin,islet beta cell function were similar in the two groups before treatment,P >0.05.After treatment,the insulin resistance index,insulin sensitivity index, fasting blood glucose,glycosylated hemoglobin and β cell function of the traditional Chinese medicine group were significantly better than those in the Western medicine group,P <0.05.Conclusion Modified Didang decoction on patients with phlegm and blood stasis type diabetes mellitus can effectively improve the β cell function and insulin resistance,reduce blood sugar levels,no serious complications,is safe and effective,and is worthy of promotion.
