肺力咳胶囊联合异丙托溴铵治疗慢性支气管炎的临床研究

Clinical study on Feilike Capsules combined with ipratropium bromide in treatment of chronic bronchitis

目的研究肺力咳胶囊联合吸入用异丙托溴铵溶液治疗慢性支气管炎的疗效。方法选取2014年12月—2017年12月江苏省人民医院接收的100例慢性支气管炎患者为研究对象,将患者随机分为对照组和治疗组,每组各50例。对照组患者雾化吸入吸入用异丙托溴铵溶液,2 mL/次,2次/d;治疗组在对照组治疗的基础上口服肺力咳胶囊,3粒/次,3次/d。两组患者均连续治疗3个月。观察两组患者的临床疗效,同时比较两组治疗前后的临床症状缓解时间、血气指标和血清炎症因子。结果治疗后,治疗组患者的总有效率为96.00%,显著高于对照组的82.00%,两组比较差异具有统计学意义(P<0.05)。治疗后,治疗组患者的咳嗽和肺部啰音消失时间显著短于对照组,两组比较差异具有统计学意义(P<0.05)。治疗后,两组患者的pH值和p O2明显升高,p CO2水平明显降低,同组治疗前后比较差异具有显统计学意义(P<0.05);治疗后,治疗组血气指标水平显著优于对照组,两组比较差异具有统计学意义(P<0.05)。治疗后,两组患者超敏C反应蛋白(hs-CRP)、肿瘤坏死因子-α(TNF-α)和白细胞介素-4(IL-4)水平均显著降低,同组治疗前后比较差异具有统计学意义(P<0.05);治疗后,治疗组血清细胞因子水平显著低于对照组,两组比较差异具有统计学意义(P<0.05)。结论肺力咳胶囊联合吸入用异丙托溴铵溶液治疗慢性支气管炎具有较好的临床疗效,可改善患者血气指标和血清炎症因子,安全性较高,值得临床推广使用。

Objective To study the effect of Feilike Capsules combined with Ipratropium Bromide Solution for inhalation in treatment of chronic bronchitis. Methods Patients(100 cases) with chronic bronchitis in Jiangsu Province Hospital from December 2014 to December 2017 were randomly divided into control and treatment groups, and each group had 50 cases. Patients in the control group were aerosol inhalation administered with Ipratropium Bromide Solution for inhalation, 2 m L/time, twice daily. Patients in the treatment group were po administered with Feilike Capsules on the basis of the control group, 3 grains/time, three times daily. Patients in two groups were treated for 3 months. After treatment, the clinical efficacy was evaluated, and the clinical symptoms remission time, blood gas indexes, and the serum cytokine levels in two groups before and after treatment were compared. Results After treatment, the clinical efficacy in the treatment group was 96.00%, which was significantly higher than 82.00% in the control group, and there were differences between two groups(P < 0.05). After treatment, the disappearance times of cough and lung rale in the treatment group were significantly shorter than those in the control group, with significant difference between two groups(P < 0.05). After treatment, pH value and p O2 in two groups were significantly increased, but p CO2 in two groups were significantly decreased, and the difference was statistically significant in the same group(P < 0.05). After treatment, the blood gas indexes in the treatment group were significantly better than those in the control group, with significant difference between two groups(P < 0.05). After treatment, the levels of hs-CRP, TNF-α, and IL-4 in two groups were significantly decreased, and the difference was statistically significant in the same group(P < 0.05). After treatment, the serum cytokine levels in the treatment group were significantly lower than those in the control group, with significant difference between two groups(P < 0.05). Conclusion Feilike Capsules combined with Ipratropium Bromide Solution for inhalation has clinical curative effect in treatment of chronic bronchitis, can improve the blood gas indexes and serum inflammatory factors, with high safety, which has a certain clinical application value.
