上海市金山区2011-2016年在管精神障碍患者死亡原因及相关因素分析

Analysis on the of death causes and relevant factors in patients with mental disorders managed in Jinshan district,Shanghai,2011-2016

目的了解上海市金山区在管精神障碍患者的死亡情况,分析其死亡原因及相关因素,为降低精神障碍患者死亡率提供科学依据。方法应用调查问卷的形式对金山区2011-2016年在管严重精神障碍死亡患者的死因及相关因素进行调查,应用SPSS22.0软件对相关资料进行统计学分析。结果2011-2016年死亡率总体平稳,女性平均死亡年龄高于男性,男性未婚率及离异率均高于女性。前5位死因顺位为神经系统疾病、恶性肿瘤、呼吸系统疾病、循环系统疾病和意外伤害。春季死亡人数最多,秋季次之,死亡地点多为家中。贫困与非贫困患者在享受基本医疗卫生服务方面没有差异,但贫困患者平均死亡年龄低于非贫困患者。结论金山区在管精神障碍患者死亡情况相对稳定,躯体疾病是导致其死亡的主要原因,但大部分患者所患躯体疾病未获得充分治疗。加强对居民开展健康教育工作,关注患者躯体健康,保障患者享受基本医疗卫生服务将是今后的重点工作。

Objective To understand the status of death of patients with mental disorders in Jinshan district in Shanghai,analyze the causes and relevant factors of death and provide effective strategy for lowering the mortality. Methods The cause and relevant factors of psychotic patients in Jinshan district were investigated from 2011 to 2016 through questionnaire,data were analyzed by the software SPSS 22. 0. Results The level of mortality over the years was stable,the average death age of women was older than men,but the unmarried and divorce rate of women were lower than those of men. The top five rank order of death causes were neurologic disorders,cancers,respiratory diseases,circulation system diseases and accidental injury.The number of people who died in spring was the largest,followed by autumn,and mostly died at home. Poor and non-poor patients accepted the equal level of medical treatment and public healthy service,but the life span of poor ones was shorter than non-poor ones. Conclusion The situation of death of patients with mental disorders managed in Jinshan district is relatively stable,main cause of which is somatic diseases,but lots of patients could not be treated adequately. To strengthen health education of citizen,pay attention to physical health of patients and ensure essential medical treatments and public health service to patients would be the important work in future.
