氯氮平联合阿立哌唑、利培酮治疗难治性精神分裂症的临床疗效对比及对患者糖脂代谢水平的影响

Comparison of clinical efficacy of clozapine combined with aripiprazole or risperidone and the influence on the level of glucose and lipid metabolism in the treatment of patients with refractory schizophrenia

目的探讨分析氯氮平联合阿立哌唑、利培酮治疗难治性精神分裂症的临床疗效及对患者糖脂代谢水平的影响。方法选取2014年4月～2016年5月期间我院收治的难治性精神分裂症患者160例,依据治疗方式的不同随机分为两组,对照组采用氯氮平联合利培酮治疗,研究组采用氯氮平联合阿立哌唑治疗。结果治疗过程中研究组患者不良反应的发生率明显低于对照组(P<0.05);研究组与对照组患者的治疗有效率分别为92.5%、78.7%,差异明显(P<0.05);治疗后对照组患者的糖脂代谢各项指标水平均明显高于治疗前(P<0.05),研究组患者的糖脂代谢各项指标水平与治疗前无明显差异(P>0.05)。结论采用氯氮平联合阿立哌唑治疗难治性精神分裂症,可以有效改善患者的精神状况,减少不良反应发生,维持糖脂代谢水平稳定。

Objective To investigate the clinical efficacy of clozapine combined with aripiprazole or risperidone and the influence on the level of glucose and lipid metabolism in the treatment of patients with refractory schizophrenia. Methods 160 patients with refractory schizophrenia treated in our hospital from April 2014 to May 2016 were randomly divided into two groups. The control group was treated with clozapine combined with risperidone,and the study group was treated with clozapine combined with aripiprazole. Results In the treatment process of the study group the incidence of adverse reaction was significantly lower than that of the control group(P<0.05). The treatment efficiency in the study group and the control group of patients with were 92.5% and 78.7%,the difference was significant(P<0.05). The glucose and lipid metabolism in patients after treatment in the control group of the was significantly higher than those before treatment(P<0.05),and those parameters in the study group showed no significant difference before and after therapy(P >0.05). Conclusion Clozapine Combined with aripiprazole in the treatment of refractory schizophrenia can effectively improve the mental status of patients,and reduce the incidence of adverse reactions as well as maintain the stability of glucose and lipid metabolism.
